젖산의 이해

Abstract

과거에 젖산은 산소 부족으로 인해 근육과 혈액에 축적되어 조직을 손상시키며 피로를 유발한다고 알려져 왔다. 또한, 생성된 젖산은 운동 후 회복기에 글라이코젠 재합성을 위한 초과 산소 소비(산소 부채, oxygen debt)의 원인이 된다고 알려져 왔다. 하지만, 동물과 사람을 대상으로 한 선행연구를 통해 젖산이 휴식과 운동 중에 에너지 요구량이 증가할 때, 적절히 에너지를 공급하여 세포 내 ATP의 농도를 정상 수치로 일정하게 유지시키는 중요한 대사물질이라는 것을 알게 되었다. 젖산은 생성된 세포 내에서 이용(세포 내 젖산 셔틀, intracellular lactate shuttle)되거나 세포간질 또는 혈관을 통해 멀리 떨어진 다른 세포로 전달되어 이용(세포 간 젖산 셔틀, cell-cell lactate shuttle)된다. 이러한 젖산 셔틀(lactate shuttle)을 통해 알 수 있듯이 젖산은 휴식과 운동 중, 운동 후 회복 시에 해당작용과 산화적 인산화 과정을 연결하는 중간 매개물질이며 글루코스 신생합성의 전구체이다. 따라서 본 연구는 휴식과 운동 중에 젖산의 생리적 기능 변화 과정과 세포 간, 세포 내 젖산 셔틀, 젖산의 임상적 활용에 대한 선행 연구를 고찰함으로써 젖산대사에 대한 최신 지견을 파악하고 그 이해도를 높이는데 그 목적이 있다.

Trans Abstract

Lactate was considered a dead-end waste product of glycolysis due to lack of oxygen. Accumulating lactate in the muscle and blood was viewed only as a major cause of muscle fatigue and tissue damage. Furthermore, buildup of lactate due to glycogen re-synthesis is thought to cause excess oxygen consumption (oxygen debt) after exercise. However, research has also shown lactate to be an important mediator in numerous metabolic processes and a fuel for aerobic metabolism. Lactate is not metabolic waste, but a major player in cellular and whole body metabolism. Lactate can be used within the cell (intracellular lactate shuttle) or be transported through the cytoplasm or blood to be used by other cells (cell-cell lactate shuttle). Lactate shuttle hypothesis has helped in identifying and understanding the link between the glyoclyic and oxidative pathways, and identified lactate as a major energy source and gluconeogenic precursor. Therefore, the purpose of this review was to better understand the physiological roles of lactate at rest and during exercise, and the concept of lactate shuttle with implications for clinical practice.

서 론

과거에 젖산은 수축하는 골격근에서 산소 부족으로 인해 생성된다고 믿어져 왔다. 해당작용의 결과로 생성되는 젖산은 근육과 혈액에 축적되며 산성증을 초래하고 조직을 손상시키며 피로를 유발한다고 알려져 왔다(Pasteur, 1863; Gould & Dye, 1932; Hill & Lupton, 1923). 또한, 생성된 젖산은 운동 후 회복기에 글라이코젠 재합성을 위한 초과 산소 소비(산소 부채, oxygen debt)의 원인이 된다고 알려져 왔다(Hill & Lupton,1923). 하지만, 오늘날 사람과 동물을 대상으로 한 수많은 연구를 통해 젖산 대사에 대한 이해가 완전히 변화되었다(Bergman et al., 2000; Bergman et al., 1999; Brooks & Donovan, 1983; Donovan & Brooks, 1983; Gertz et al., 1988; Mazzeo et al., 1986; Stanley et al., 1985; Stanley et al., 1986; Stanley et al., 1988). 즉, 젖산은 더 이상 피로물질이 아닌 에너지원으로서 세포 내 ATP(아데노신3인산, adenosine triphosphate) 항상성 유지에 도움을 주는 대사과정의 중요한 매개물질이며 글루코스 항상성 유지에 기여한다.

젖산은 산소가 충분한 휴식과 운동 중에도 계속해서 생성되고 이용되는데, 휴식 시에는 생성된 젖산의 약 50%, 운동 중에는 약 75%가 산화되어 에너지원으로 이용되고, 나머지는 글루코스 신생합성(gluconeogenesis, GNG)의 전구체(precursor)로 이용된다. 따라서 젖산은 생성된 세포 내에서 이용(세포 내 젖산 셔틀, intracellular lactate shuttle)되거나 세포간질 또는 혈관을 통해 멀리 떨어진 다른 세포로 전달되어 이용(세포 간 젖산 셔틀, cell-cell lactate shuttle)된다. 이러한 젖산 셔틀(lactate shuttle)을 통해 알 수 있듯이 젖산은 해당작용과 산화적 인산화 과정을 연결하는 중간 매개물질이다.

젖산 셔틀 개념은 외상성 뇌손상(traumatic brain injury, TBI) 및 종양학(암) 등과 같은 분야에 임상적 활용을 위해서 여러 연구(Brooks & Martin, 2014; Glenn et al., 2015; Sonveaux et al., 2008; Vander Heiden et al., 2009)가 수행되고 있다.

따라서 본 연구는 휴식과 운동 중에 젖산의 생리적 기능 변화 과정과 세포 간, 세포 내 젖산 셔틀, 젖산의 임상적 활용에 대한 선행 연구를 고찰함으로써 젖산대사에 대한 최신 지견을 파악하고 그 이해도를 높이는데 그 목적이 있다.

젖산 개념의 변천과정

전통적으로 젖산은 운동 중 산소 부족으로 인하여 근육과 혈액에 축적되어 산성증과 피로를 유발하는 물질로 여겨져 왔으나, 오늘날 젖산은 산소의 존재와 상관없이 항상 생성, 제거 또는 이용되며 우리 몸의 주요 에너지원으로서, 뿐만 아니라 글루코스 항상성 유지를 위한 중요한 대사물질로 간주되고 있다.

젖산 개념의 변천 과정은 젖산의 생성과 제거의 원인과 결과로 설명할 수 있다. 19세기 프랑스의 화학자 파스퇴르는 발효의 연구로 젖산을 발견하고 발효의 의의를 확립하였다. 그 당시 파스퇴르는 산소부족의 결과로 젖산(lactic acid)이 생성된다고 보고하였으며, 산소 존재 여부에 따라 해당작용이 빠르게 혹은 느리게 진행된다고 믿었다(Pasteur, 1863). 20세기에 이르러 Fletcher & Hopkins(1907)는 수축하는 근육에서 젖산이 생성되고, 생성된 젖산의 제거에는 산소가 필요하다는 결과를 보고하였다. 이 후 Meyerhof(1920a, 1920b, 1920c)는 혈액순환과 산소의 공급을 차단한 개구리 근육을 이용하는 동물 실험을 통해 젖산의 전구체가 글라이코젠이라는 것을 확인하였다. 수축하는 개구리 근육에서 글라이코젠이 분해되고 젖산이 생성되었으며, 회복 시에는 운동 중 생성된 젖산의 일부(1/3)는 산화되고, 대부분(2/3)은 다시 글라이코젠으로 전환된다는 사실도 밝히게 되었다.

그 뒤를 이어 Hill & Lupton(1923)은 운동이 시작되거나 최대 강도로 운동을 하게 되면 소비할 수 있는 산소가 부족하다는 것을 인지하면서 무산소성 대사를 설명하기 위해 산소부채를 측정하였다. 산소부채는 운동 후 회복기에 휴식 수준 이상으로 소비되는 산소로 무산소성 대사 추정에 이용해 왔다. 그 뒤를 이어 Margaria et al.(1933)은 운동 후 회복기의 에너지 대사 연구에서 다음과 같은 연구 결과를 보고하였다. 즉, 고강도 운동 후 회복기에 혈중 젖산 농도는 즉시 감소하지 않고 일정 시간 유지된다는 것이다. 따라서 운동 후 회복 초기에는 젖산 농도의 변화와 관련 없는 휴식 수준 이상의 초과 산소 소비가 일어난다고 하여 이를 비젖산성(alactacid)이라 하였다. 뿐만 아니라 회복기에 젖산 농도의 감소와 관련된 초과 산소 소비를 젖산성(lactacid)이라 하였다.

그러나 무산소 대사 추정을 위해 산소부채를 측정하는 것이 적절치 않다는 것이 Bang(1936)의 연구를 통해 밝혀졌다. 동일 강도에서 서로 다른 시간대의 사이클 운동 전, 중, 후에 젖산 농도의 변화를 관찰하였는데, 젖산의 농도는 운동 중과 운동 후 회복기에 상승 또는 감소하는 다양한 반응을 나타내면서 젖산이 운동 후 회복기 초과 산소 소비(산소부채)를 결정한다는 선행연구 결과를 반박하였다.

산소 부족이 젖산의 생성과 축적에 기인한다는 기존의 이해를 반박하는 또 다른 예는 젖산 역설(lactate paradox)로 설명이 가능하다. 젖산 역설은 고지대의 저산소 환경으로의 갑작스러운 노출(acute)과 일정 기간의 적응 후(chronic)에 동일한 절대 강도의 운동 중 젖산 농도에 차이가 있었다는 것이다. 저산소 상황에서는 항상 젖산 농도가 높아야 하는 것이 기존의 이해이지만, 일정 기간 저산소 환경에서 적응한 후에는 젖산의 농도가 낮게 나타났다는 것이다(Reeves et al., 1992).

이어 개를 모델로 한 실험연구에서 개의 다리 근육이 수축할 때, 젖산이 생성되는 것을 보고하였고(Stainsby & Welch, 1966), 방사선 동위원소([U-14C])를 이용한 젖산 대사 연구에서는 다음과 같은 중요한 결과를 보고하였다(Depocas et al., 1969). 첫째, 12시간 공복 후 휴식상태에서도 젖산이 생성되고 이용되며, 둘째, 휴식 중에 생성된 젖산의 50%가 산화로 제거되며, 셋째, 안정 상태에서 운동 중 혈중 젖산 농도의 변화는 크지 않으며, 넷째, 운동 중에는 산화로 제거되는 젖산이 75%로 증가한다는 것, 마지막으로 운동 중 생성된 젖산의 10-25%가 코리 사이클(Cori cycle)에 의해 글루코스로 전환된다는 것이었다. 즉, 휴식 중에도 젖산이 생성되며, 휴식 중에 생성된 젖산 중에서 1/2은 산화된다는 결과였다. 운동 중 젖산의 생성률과 이용률(동역학)은 휴식 상태에 비해서 증가하는데 운동 중 산화로 제거되는 젖산은 3/4으로 증가한다고 보고하였다.

또한, 쥐를 모델로 한 Brooks & Gaesser(1980)의 연구에서 탈진 운동 후 회복기에 글라이코젠 농도가 운동 직후 증가하지 않았고, 탈진 운동 후 회복기에 젖산은 대부분이 산화된다는 결과를 보고하였다. 구체적으로 운동 후 회복기에 쥐의 혈액, 간, 신장, 심장, 근육 등에 방사성 동위원소([U-14C])를 이용하여 젖산의 대사 경로를 살펴본 결과, 젖산의 대사 경로는 다양하다는 것을 보고하였으며, 젖산은 주로 운동 후에 산화되지만 몇몇 다른 경로에도 유입되는 것을 발견하였다. 포유류의 백근에서 약간의 젖산이 생체(in situ) 상태에서 젖산이 글라이코젠으로 전환되고 양서류 파충류와는 달리 대부분의 젖산이 운동 중 제거되거나 회복기에 산화되었다. 순환계가 발달된 포유류는 운동 후 많은 양의 젖산이 근육에서 생성되어 간과 같은 다른 조직에서 제거된다는 것이었다(Brooks & Gaesser, 1980).

뿐만 아니라, 사람을 대상으로 활동근의 산소분압(Po2)과 젖산 방출(lactate efflux)에 대한 관계 연구(Richardson et al., 1998)에서 젖산은 휴식과 운동 중 산소가 존재할 때(유산소 조건) 형성된다는 것을 다시 한 번 증명하였다. 해수면에서 휴식 중, 건강한 성인에 대한 근육의 산소분압은 약 40 torr이고, 운동 시작 후 최대산소소비량(Vo2 max)의 50%에 해당하는 운동 강도에 이르기까지 근육 세포 내의 산소분압은 약 4 torr까지 급격히 떨어지게 된다. 운동 강도가 점차 증가하여 최대에 도달할 때까지도 근육 세포 내의 산소분압은 미토콘드리아가 최대호흡율에 도달하기 위해 필요한 산소 분압인 1 torr이상으로 유지된 것을 발견하였다. 반면, 근육의 젖산은 근육 세포 내의 산소분압과는 관계없이 방출되었는데, 휴식과 최대산소소비량의 65%에 해당하는 운동 강도에 이르기까지는 소량의 젖산이 방출되었고, 최대산소소비량의 65% 이상의 운동 강도에서는 동맥에서 에피네프린 농도의 증가와 함께 근육에서 젖산의 방출이 급격히 증가하였다<Fig. 1>.

최대산소소비량의 65%이상의 운동 강도에서 혈중 젖산은 급격히 증가하게 되는데 이것은 몇 가지 요인으로 설명할 수 있다. 근수축 자체가 글라이코젠의 분해와 젖산의 생성을 자극하거나, 호르몬에 의해서 글라이코젠 분해와 해당작용이 가속화 될 경우, 또는 젖산을 제거하는 글루코스 신생합성 조직으로부터 젖산을 생성하는 해당성 조직으로의 혈류 재분배 등은 더 많은 파워를 요구하는 고강도 운동 중 혈중 젖산 증가를 가져온다. 다시 말해서, 운동 중 혈중 젖산의 증가는 근육과 다른 조직에서의 젖산 생성을 감당할 만한 제거 능력이 없기 때문이다(Richardson et al., 1998).

젖산 농도는 운동의 강도와 시간에 영향을 받는다. 그리고 트레이닝(운동 훈련)은 젖산 농도와 젖산 전환율에 영향을 미친다. 운동의 강도가 증가하게 되면 혈중 젖산 농도가 증가하게 되며, 저강도와 고강도 운동 모두에서 트레이닝을 받지 않은 그룹보다 트레이닝을 받은 그룹에서는 낮은 젖산 농도를 나타내었다. 하지만 젖산 전환율은 트레이닝의 영향을 받지 않는 것으로 나타났다(Donovan & Brooks, 1983).

동일한 운동 수행 시, 운동 시간이 증가하게 되면 혈중 젖산 농도가 증가하다가 10분 이상 지속될 경우, 혈중 젖산 농도는 운동 시작에 비해 떨어진다. 트레이닝을 받은 경우, 동일한 강도의 운동 수행 시 트레이닝을 받지 않은 그룹에 비해 혈중 젖산 농도는 상승하지 않았으며, 운동 시간이 지속될수록 트레이닝을 한 그룹은 젖산 농도가 빨리 감소하며 트레이닝을 받지 않은 그룹보다 낮은 수준으로 유지되었다. 트레이닝 그룹의 낮은 젖산 농도는 트레이닝의 효과로 인체의 젖산 제거 능력이 향상되었기 때문이라고 설명할 수 있다(Mazzeo et al., 1986).

세포 간 젖산 셔틀(Cell-Cell Lactate Shuttle)

초기에 젖산 셔틀은 동물(Brooks & Donovan, 1983; Donovan & Brooks, 1983)과 사람(Bergman et al., 2000; Bergman et al., 1999; Mazzeo et al., 1986; Stanley et al., 1985; Stanley et al., 1986; Stanley et al., 1988)을 대상으로 한 글루코스와 젖산의 동역학 연구의 결과를 토대로 발전하였다. 젖산 셔틀은 휴식과 운동 중에 세포간질 또는 혈관을 통해 순환하는 젖산이 산화되고 글루코스 신생합성의 전구체를 공급한다는 것을 의미한다. 뿐만 아니라 활동근에서 생성된 젖산은 운동 중에 심장에서도 주요 에너지원으로 사용된다(Gertz et al., 1988).

해당작용이 빠른 근육세포(FG, IIb 또는 IIx형)에서 생성된 젖산은 다른 산화 능력이 높은 근육세포(SO, I형)로 전달되어 산화된다. 또한 정맥혈로 전달된 젖산은 활동근으로 재순환할 수 있으며, 산화될 수 있다. 이와 같이 생성된 젖산이 이동하는 것을 ‘세포 간 젖산 셔틀’이라 한다<Fig. 2>.

휴식과 낮은 강도의 운동 중 젖산은 생성과 제거/소비가 같은 비율로 이루어지며, 운동을 시작하게 되면 산소 소비가 증가하고 젖산의 생성과 제거가 균형(전환, turnover)을 이룰 때까지 근육으로부터 많은 젖산이 나오게 된다. 운동과 함께 젖산의 전환은 매우 빠르게 나타나지만, 젖산의 제거/소비 속도가 생성 속도가 같다면 혈중 젖산 농도는 일정하게 유지된다. 운동 중 젖산의 전환율은 휴식 시보다 높다. 운동 중 세포 내 젖산 순환은 젖산을 제거하는 데 있어 세포간 젖산 순환에 의해 도움을 받는다.

선행하는 연구의 결과를 종합해보면, 휴식과 운동 중에 젖산은 항상 빠르게 생성되고 이용된다(Brooks et al., 2005). 즉 휴식 시에는 생성된 젖산의 50%, 운동 중에는 약 75%가 산화되어 에너지원으로 이용되고, 나머지는 글루코스 신생합성의 전구체로 이용된다<Fig. 3>. 뿐만 아니라 젖산의 동역학(생성과 제거)과 산화가 글루코스에 비해 유사 또는 능가한다고 보고하고 있다(Brooks et al., 2005).

젖산 셔틀의 기본 가정은 젖산의 생성과 제거 및 이동이 세포와 조직에서 이루어진다는 것이다. 동물 실험에서 같은 근육의 백근보다, 심지어 동맥혈관보다 근접한 적근에서 젖산의 농도가 낮은 것을 발견하였고, 젖산이 적근과 백근에서 이동이 이루어지는 것을 관찰하였으며, 마찬가지로 사람의 골격근에서도 젖산의 이동/활용을 발견하였다(Baldwin et al., 1977; Stanley et al., 1986). 세포에서 일어나는 젖산의 유입은 일반적으로 세포간 젖산 셔틀로 젖산 수송 단백질(monocarboxylate transport proteins, MCTs)을 수반하고 젖산의 농도와 수소 이온 농도의 차이에 의한 촉진적 교환(facilitated exchange)에 의해 일어난다.

지구성 트레이닝은 운동 중에 젖산의 생성에 거의 영향을 미치지 않았으나 산화와 글루코스 신생합성에 의한 젖산의 제거(제거율=젖산제거/젖산농도)는 증가시킨다고 보고하고 있다. 고강도 운동에서도 혈중 젖산 제거율은 증가하며, 이는 젖산의 제거가 그 농도에 비례하며, 더 빠른 제거를 위해서는 더 많이 생성되어야 하는 것을 의미한다. 서로 다른 운동 상황에서 젖산은 초기에 증가하다가 휴식/안정 시 수준으로 낮아지기도 하고, 운동 중에 계속해서 상승하기도 한다. 혈중 젖산의 감소는 생성보다 제거가 많을 때이며, 증가는 생성이 제거보다 많을 때이다. 지구성 트레이닝으로 인한 젖산의 제거율 증가는 세포막과 미토콘드리아의 젖산 수송 단백질-1(MCT-1)의 발현 증가와 미토콘드리아 단백질 증가에 기인한다(Dubouchaud et al., 2000).

Dubouchaud et al.(2000) 의 연구는 사람의 외측 광근(vastus lateralis muscle)을 이용하여 장기간 지구성 트레이닝이 MCT 동형단백질(MCT-1, MCT-4)의 발현에 미치는 영향을 최초로 조사하면서 MCT-1과 MCT-4는 세포 간 젖산 셔틀에 참여하고 특히 MCT-1은 세포 내 젖산 셔틀을 용이하게 한다는 결과를 보고하였다<Fig. 4>. 운동 중 젖산 셔틀은 많은 연료를 제공할 수 있으며, 근육 세포막에 있는 젖산 수송 단백질(MCT-1과 MCT-4)은 젖산의 방출과 유입을 촉진시킨다.

세포 내 젖산 셔틀(Intracellular Lactate Shuttle)

최근 들어 세포 내 젖산 셔틀의 존재가 많은 연구를 통해 확인되고 있다. 세포 내 젖산 셔틀이란 한 세포에서 생성된 젖산이 동일한 세포 내에서 미토콘드리아로 이동해서 산화된다는 것이다. Brooks et al.(1991)Brooks et al.(1999) 의 연구에 의하면, 심장, 골격근, 간의 미토콘드리아에서 젖산이 직접 산화된다고 보고하면서 동일 세포 내에서 젖산의 산화를 위해서는 미토콘드리아 젖산 탈수소효소(lactate dehydrogenase; LDH 또는 LDHi)와 젖산 수송 단백질이 필요하다는 것을 확인하였다.

최신 기술을 활용한 Pagliaini et al.(2008) 의 연구에서도 미토콘드리아에 존재하는 LDH와 MCT-1을 확인하였다. 역사적으로도 젖산 셔틀을 최초로 추측한 Baba & Sharma(1971)의 전자현미경을 이용한 조직화학적 연구에서 LDH가 쥐의 심근과 흉근의 미토콘드리아에 존재하는 것을 발견하였다. 세포 내 젖산 셔틀은 LDH 동형단백질(LDH-C)의 존재와 정맥의 미토콘드리아의 젖산 산화와의 관련성을 밝힌 Hochachka(1980)에 의해서 최초로 묘사된 것으로 추정하고 있다. Kline et al.(1986)Brandt et al.(1987) 의 연구에서도 쥐의 간, 신장, 심장의 미토콘드리아에서 LDH가 존재하는 것을 확인하였다. 해당작용으로 생성된 젖산과 피루브산이 세포질에서 미토콘드리아의 내부로 이동되고 미토콘드리아 내부에도 LDH가 존재하며 젖산이 탈수소되어 피루브산이 된다고 보고하였다. 여기서 젖산과 피루브산이 미토콘드리아 막을 통과하기 위해 수송체가 필요하다는 것을 밝혔으며, 세포 내 젖산의 이동을 위해서는 세포막의 수송체인 젖산 수송 단백질이 필요한 것을 밝히게 되었다. 젖산 수송 단백질-1(MCT-1)은 골격근과 심장근의 미토콘드리아에 풍부하게 존재한다(Brooks et al., 1999).

그 이후 많은 연구에서 미토콘드리아 내에서 LDH가 존재하는 것을 발견하였다(Dubouchaud et al., 2000; Hashimoto et al., 2006; Hashimoto et al., 2008; Hashimoto et al., 2005; Pagliarini et al., 2008; Passarella et al., 2008). 또한, Kline et al.(1986) 은 분리된 간의 미토콘드리아에서 젖산이 피루브산만큼 산화가 빠르다는 것을 추가로 보고하였으며, Brooks et al.(1991)의 연구에서는 분리된 근육의 미토콘드리아에서 젖산이 산화되는 것을 확인하였다. 젖산의 수송과 확산을 촉진시키고 이동시키기 위해서는 수소이온과 젖산 농도의 차이가 필요하고, 젖산은 산화와 글루코스 신생합성으로 이용되기 때문에 활발하게 호흡하는 미토콘드리아는 젖산 셔틀이 작용하기 위해서 필수적이다. 세포솔에 존재하는 젖산은 MCT를 통해 미토콘드리아로 들어가게 되고 미토콘드리아 내의 LDH에 의해 피루브산으로 되고 TCA(트리카르복실산, tricarboxylic cycle) 회로에 의해 산화되는 것이 세포 내 젖산 셔틀이다<Fig. 5>.

세포 내 젖산 셔틀은 다음과 같은 여러 생리적 현상을 예측한다. 첫째, 해당작용은 세포솔 LDH의 최대속도(Vmax)가 다른 해당작용의 경로에 관여하는 효소보다 높고 평형상수(Keq,3.6x104mol-1)가 크기 때문에 필연적으로 젖산을 생성한다. 둘째, 안정상태에서 동맥혈의 젖산 농도 증가는 근육에서 젖산의 섭취를 증가시킨다(Gertz et al., 1988). 셋째, 지구성 트레이닝으로 인한 높은 젖산 제거율은 운동 중에 젖산의 생성이 증가하더라도 근육과 혈액에 젖산의 축적을 감소시킨다(Donovan & Brooks, 1983; Donovan & Pagliassotti, 1989; Stanley et al., 1988). 따라서 트레이닝으로 인한 미토콘드리아의 밀도 증가는 젖산의 수송 및 산화효소를 증가시켜 세포 내 젖산 셔틀을 촉진시킨다.

젖산의 임상적 활용

최근 젖산은 뇌 손상 후에 글루코스 대사가 감소할 때<Fig. 6> 뇌 신경 대사에 직접적으로 사용되는 대사물질이라고 알려지고 있다(Brooks & Martin, 2014). 신경전달과정에서 신경 연료로 사용되며, 특히 외상성 뇌손상 환자의 관리와 치료의 중요한 물질로 사용되고 있다(Brooks & Martin, 2014; Glenn et al., 2015). 또한, 암의 치료에 있어서 젖산 셔틀의 구성요인인 MCT와 LDH를 차단함으로써 암세포의 성장을 억제할 수 있는 가능성을 보여주고 있다.

일반적으로 뇌손상 환자는 대사적 위험에 노출되어 있다(Stein et al., 2012; Vespa et al., 2012). 뇌 손상 후, 뇌 대사는 감소하게 되는데 젖산은 뇌의 병태생리학적으로 중요한 역할을 하는 것으로 보고되고 있다(Brooks & Martin, 2014). 뇌 손상 이후에는 뇌의 글루코스 대사가 감소함에도 불구하고 젖산은 젖산 셔틀을 통해 뇌의 에너지 및 글루코스 전구체로 이용됨으로써 뇌의 글루코스 대사에 많은 영향을 미친다는 연구 결과가 보고되었다(Brooks & Martin, 2014). 외상성 뇌손상 환자의 연구(Brooks & Martin, 2014; Glenn et al., 2015)에서 안정동위원소 추적자로 측정된(trace-measured) 젖산이 직접적으로 산화되거나, 글루코스 신생합성의 전구체로 젖산이 이용되어 간접적으로 뇌에 글루코스를 공급해 준다는 결과를 보여주었다. 좀 더 구체적으로 살펴보면, 외상성 뇌손상 이후 뇌의 총 탄수화물 대사가 감소하였지만, 탄수화물의 구성요인인 젖산 및 글루코스의 기여정도는 건강한 정상인(젖산 9% vs. 글루코스 91%)과 외상성 뇌손상 환자(젖산 10% vs. 글루코스 90%)에서 거의 비슷한 비율로 나타났다. 주목할 점은 건강한 정상인에 비해서 외상성 뇌손상 환자의 경우 글루코스 신생합성을 통한 글루코스 공급은 현저하게 증가(약 4배)하는 것에 비해 글라이코젠 분해를 통한 글루코스 공급은 감소(약 2배)하는 것을 볼 수 있다(건강한 정상인 GNG 15% vs. GLY 76%, 외상성 뇌손상 환자 GNG 58% vs. GLY 32%). 젖산이 글루코스 신생합성에 중요한 전구체임을 고려할 때, 외상성 뇌손상 환자의 현저한 글루코스 신생합성의 증가(약 4배)는 젖산 셔틀이 외상성 뇌손상 환자에게 중요하게 작용되고 있음을 보여주는 결과임을 알 수 있다. 뿐만 아니라 젖산의 혼합물이나 보충제는 손상된 뇌에 영양을 제공할 수 있을 것으로 예측된다(Glenn et al., 2015).

젖산 셔틀 개념은 항암치료에도 활용될 가능성이 제기되고 있다. Warburg(1956)에 의하면 암세포는 성장하면서 혈관과 멀어짐으로서 주로 해당과정을 통해 세포에 에너지를 공급받는다는 결과를 보고하였다. 따라서 암세포는 정상세포보다 글루코스 흡수율 및 해당작용 속도가 매우 빠르다. 이러한 점에 착안하여 젖산 셔틀의 구성요인인 MCT와 LDH의 차단이 항암치료에서 종양을 제거하는 가능성을 제기하기도 하였다(Sonveaux et al., 2008).

결론

젖산은 이제 더 이상 산소 부족의 결과로서 생성되는 단순한 피로물질이거나 노폐물이 아니다. 동물과 사람을 대상으로 한 선행연구를 통해 젖산이 피루브산 이상으로 해당작용과 산화적 대사 사이를 연결하는 대사물질이라는 것을 알게 되었다. 또한, 휴식과 운동 중에 에너지 요구량이 증가할 때, 적절히 에너지를 공급하여 세포 내 ATP의 농도를 정상 수치로 일정하게 유지시키는 중요한 대사물질이다. 뿐만 아니라 젖산은 글루코스 신생합성을 위한 전구체로서 혈중 글루코스 농도를 정상으로 유지시켜 우리 몸을 운동이라는 자극에 대하여 적절하게 대응하고 도움을 주는 주요한 대사 물질임을 알게 되었다. 최근에는 젖산 셔틀의 개념이 외상성 뇌손상과 암 등에서 임상적 활용을 위한 여러 연구가 진행되고 있으며, 그 적용 범위를 넓히고 있다.
