Abstract

이 연구는 12주간의 저항성운동이 20대 마른 비만(normal weight obesity) 여성의 체격, 신체구성, 인슐린 저항성 및 혈중지질에 미치는 영향을 규명하는 것이다. 마른 비만 기준에 해당되는 16명의 대상자를 운동 집단 8명과 통제 집단 8명으로 무선 할당하였다. 운동 집단의 대상자는 12주간 주3회 저항성운동을 실시한 반면, 통제 집단의 대상자는 동일한 처치 기간 중 평소의 생활을 유지하도록 하였다. 사전검사와 사후검사에서 체격, 신체구성, 인슐린 저항성, 그리고 혈중 지질을 측정하여 두 집단 간에, 그리고 두 검사 간에 반복 이원변량분석(repeated two-way ANOVA)을 이용하여 분석하였으며, 이 연구의 주요 결과는 다음과 같다. 1) 체중, BMI, 허리둘레, 엉덩이둘레, WHR과 WHtR이 운동 집단에서 유의하게 감소되었다. 2) 신체구성 변인의 유의한 변화는 없었으나, 운동 집단의 체지방량이 통제 집단에 비하여 더 감소되는 경향을 보였다. 3) 공복혈당은 두 집단에서 모두 유의하게 감소되었으나, 다른 인슐린 저항성 변인에서는 유의한 변화가 없었다. 4) 모든 혈중 지질 변인에서 유의한 변화가 나타나지 않았다. 따라서 12주간의 저항성운동이 20대 마른 비만자의 체격과 신체구성에 긍정적인 영향을 미쳤으나 인슐린 저항성과 혈중 지질에는 영향을 미치지 못했던 것으로 판단된다. 향후 연구에서 보다 높은 연령의 마른 비만자를 대상으로 연구를 진행한다면 저항성운동이 대사 지표에 미치는 영향을 보다 명확하게 규명할 수 있을 것이다.

Trans Abstract

This study was designed to examine the effects of 12 weeks of resistance exercise training on physique, body composition, insulin resistance, and blood lipid profiles in 20s normal weight obese females. Sixteen females were randomized into one of following two groups: resistance training group (RT group; n=8) and control group (CON group; n=8). Subjects in RT group completed 12 weeks of resistance exercise training for three times/wk, and subjects in CON group were asked to maintain their normal life pattern during the same intervention period. Data were analyzed using two-way repeated measures ANOVA with post hoc test. Main results of the present study were as follows: 1) Body weight, BMI, waist circumference, hip circumference, WHR, and WHtR decreased significantly in RT group. 2) All variables regarding body composition did not change in both groups; however, fat mass was tended to decrease more in RT group than CON group. 3) Fasting plasma glucose decreased significantly in both groups, whereas other variables regarding insulin resistance did not change significantly in both groups. 4) All variables regarding blood lipids did not change significantly in both groups. Results indicate that 12 weeks of resistance exercise training was beneficial in physique and body composition; however, it was not beneficial in insulin resistance and blood lipid profiles in 20s normal weight obese females. Future research including normal weight obese subjects with higher age would be warranted to elucidate more clearly the effects of resistance exercise training on metabolic status.

서 론

WHO(2015)에 의하면 2014년을 기준으로 전 세계 성인의 38%가 과체중자이고, 13%가 비만자인 것으로 조사되었다. 비만 인구는 식생활의 서구화와 좌식생활 습관에 의하여 더욱 증가되고 있다. 국내의 경우 체질량지수(body mass index: BMI)를 기준으로 비만에 해당하는 성인은 24.5%인 것으로 조사되었으며, 주5일 이상 하루 30분 이상 걷는 비율은 불과 38.2%로 신체활동량 저하에 인한 비만 발생이 계속적으로 증가하고 있는 추세이다(Ministry of Culture, Sports and Tourism, 2014).

비만이 위험한 이유는 다양한 만성질환과 대사질환의 주요 원인이 되기 때문이다(Dandona et al., 2005). 비만으로 인한 지방조직의 과다 축적은 염증 반응을 수반하고 인슐린 저항성을 악화시켜 제2형 당뇨병의 발병률을 높일 뿐만 아니라(De Heredia et al., 2012) 고지혈증의 위험을 높이고(Sullivan et al., 2008), 고혈압을 유발하기도 한다(Kotsis et al., 2010). 비만으로 인하여 발생한 질환은 동맥경화증(atherosclerosis)과 같은 심혈관계 질환의 발생률을 높여(Mathieu et al., 2010) 결과적으로 생명을 위협하기도 한다. 이와 같이 비만은 각종 대사질환을 초래하고 사망률까지 증가시키기 때문에 매우 중요한 질병 상태로 인식해야 한다.

비만을 평가하는 방법에는 여러 가지 지표가 존재하며, 그 중 BMI를 이용하는 평가 법은 사용방법이 매우 간단하여 가장 보편적으로 이용되고 있다. WHO (2015)는 BMI 수치가 25 ㎏·m-2 이상인 경우를 과체중으로 판정하고 있으나, 서양인과 아시아인의 차이가 존재하기 때문에 국내에서는 BMI 25 ㎏·m-2 이상을 비만으로 판정하고 있다(Korean Society for the Study of Obesity, 2012). 그러나 BMI는 단순히 신장과 체중으로 비만 수준을 평가하기 때문에 제지방량과 체지방량의 구분이 어려워 비만 판정이 정확하지 못하다는 제한점을 갖고 있다. 따라서 BMI 수치와 함께 체지방률을 이용하여 비만을 평가하는 것이 비만을 판정하는데 보다 더 타당한 것으로 알려져 있다(Ko & Kim, 2007).

비만의 심각성이 끊임없이 제기되고 있는 가운데, 최근 국내에서 주로 여성에게 있어 마른 비만(normal weight obesity)의 문제가 부각되고 있다(Chung, 2009). 마른 비만은 BMI가 25 ㎏·m-2 미만으로서 체중은 정상 범위에 속하지만 체지방량이 과다한 상태를 말한다. 마른 비만 인구의 증가 문제는 날씬한 몸매를 선호하는 우리나라 여성의 성향과 외모에 대한 비정상적인 관심이 더해진 결과로, 규칙적인 운동을 통한 체중 감량보다 부적절한 다이어트 방법을 이용하기에 더욱 가중된다. 보고에 따르면 국내 여성의 활동량은 계속적으로 감소하고 있으며(Centers for Disease Control and Prevention, 2010), 일주일에 2회 이상 운동하는 20대 여성은 34.1%에서 26.7%으로 급감하였으며 전혀 운동을 하지 않는 비율도 59.5%에서 67.3%로 크게 증가하였다(Ministry of Culture, Sports and Tourism, 2010; Ministry of Culture, Sports and Tourism, 2012). 이와 같은 종합적인 현상들이 근육량이 낮고 체지방량은 높은 상태인 마른 비만의 인구를 증가시키는 요인으로 작용하고 있다(Lim & Seo, 2010).

마른 비만은 비만율에 대한 통계를 집계할 때 포함되지 않지만, 그 위험성은 일반 비만과 거의 같은 수준인 것으로 평가된다(De Lorenzo et al., 2007). 이와 관련하여, Succurro et al.(2008)은 마른 비만자가 정상인에 비해 인슐린 저항성이 더 높고 제2형 당뇨병 위험성이 더 크다고 보고하였으며, Madeira et al.(2013)은 마른 비만자의 고밀도 지단백 콜레스테롤(high density lipoprotein cholesterol: HDL-C) 농도가 낮고 중성지방(triglyceride: TG) 농도는 높아 대사질환 위험성이 높다고 보고하였다. 뿐만 아니라 마른 비만자는 일반 비만자과 같이 혈압이 높고, 상대적으로 높은 내장지방면적과 복부비만 정도를 보이며(Dvorak et al., 1999; Conus et al., 2007), 대사질환의 양상이 일반 비만자와 거의 같아 심혈관계 질환 발병 위험이나 심혈관계 질환으로 인한 사망 위험도 높은 것으로 조사되었다(Romero et al., 2010). 아울러 총콜레스테롤(total cholesterol: TC), 저밀도 지단백 콜레스테롤(low density lipoprotein cholesterol: LDL-C), 그리고 HDL-C의 비율을 이용하여 심혈관질환 위험을 예측하는 동맥경직도의 수준 역시 마른 비만자와 일반 비만자가 서로 유사한 것으로 나타나(De Lorenzo et al., 2007), 마른 비만자의 심혈관 질환 위험 수준이 일반 비만자 못지않게 높은 것을 알 수 있다. 따라서 일반 비만자뿐만 아니라 마른 비만자도 대사질환에 특별한 경각심을 가져야 하며, 외형상 정상 체중으로 보이는 것에 만족하지 않고 지속적으로 건강 관리를 할 필요가 있다.

저항성운동은 근육량이 적고 상대적으로 지방량이 많은 비만 여성에게 적절한 운동 방법 중 하나로 인식되어 왔다. 저항성운동은 뼈의 미네랄 밀도를 높여 골다공증 위험을 예방하고(Kelley et al., 2001) 제지방량을 높인다(Olson, 2006). Strasser & Schobersberger(2011)는 저항성 트레이닝이 비만자의 체지방량을 감소시킬 뿐만 아니라 운동과 식이 제한에 따라 줄어든 지방량을 유지시키고, 비정상적인 당대사로 인한 당화혈색소(HbA1c)를 감소시키며, 대사질환 위험요소 또한 감소시킨다고 보고하였다. Castaneda et al.(2002)은 16주간 주3회 실시한 점진적 저항성운동이 제2형 당뇨병 환자의 혈당 조절에 효과적이었다고 보고하였으며, Yoo & Jo(2009)는 마른 비만 여대생을 대상으로 8주간 복합운동 프로그램을 실시한 결과, 제지방량이 증가되고 체지방량은 감소되었다고 보고하였다. 특히, 마른 비만 여성의 운동 목적이 체중 감량보다 체지방 감소와 근육량 증가인 것으로 보고됨에 따라(Chung, 2009), 마른 비만 여성에게 저항성운동은 효율적인 운동법이 된다는 것을 알 수 있다.

전술한 내용을 종합하면, 마른 비만은 BMI는 정상이지만 체지방률이 높아 비만으로 분류되는 것으로서, 외형상 날씬해 보인다는 이유로 건강 관리에 소홀할 가능성이 큰 상태이다. 마른 비만은 근육량이 적다는 특성을 보이며, 일반 비만 못지않게 각종 대사질환에 노출될 가능성이 크다. 따라서 마른 비만자를 대상으로 근육량을 증가시키는데 적합한 저항성운동 트레이닝을 실시하여 그 효과를 규명하는 시도가 필요하다고 판단된다. 마른 비만 여성을 대상으로 저항성운동을 실시하여 대사관련 변인을 관찰한 종단적 연구는 거의 전무한 실정인 가운데 이 연구에서는 마른 비만자를 대상으로 12주간 저항성운동을 처치하여 신체구성과 대사관련 혈액성분에 미치는 영향을 규명하고자 하였다.

연구 방법

연구 대상자

연구의 대상자는 20대 여성으로서 BMI 25 ㎏·m-2 미만이고 체지방률이 28% 이상인 마른 비만자 16명이었다. 대상자는 최근 6개월간 운동 트레이닝을 수행하지 않았으며, 특별한 형태의 식이를 하지 않는 자들이었다. 선정된 16명의 대상자를 저항성운동 트레이닝을 수행하는 운동 집단 8명과 평소의 생활습관을 유지하며 특별한 처치를 하지 않는 통제 집단 8명으로 무선할당 하였다. 연구 진행에 앞서 오리엔테이션을 실시하여 검사와 처치에 관한 내용을 설명하였다. 연구의 내용을 이해하고 자발적으로 참여하고자 하는 자원자로부터 검사동의서를 받은 후에 연구에 참여하도록 하였다. 연구 대상자의 신체적 특성은 <Table 1>에 제시된 바와 같다.

측정 항목과 방법

이 연구에서는 대상자를 운동 집단과 통제 집단으로 구분한 후, 두 집단 간에 체격, 신체구성, 인슐린 저항성, 그리고 혈중 지질을 비교ㆍ분석하였다. 이 연구에서 측정한 항목과 구체적인 측정 방법은 다음과 같다.

1) 체격과 신체구성

신장은 수동식 일반 신장계(Samhwa, 한국)를 이용하여 측정하였다. 신발을 벗고 가벼운 복장을 한 뒤, 시선은 전방을 보도록 한 후 턱을 빼고 눈과 귀를 연결하는 선이 수평이 되도록 하고 똑바로 선 자세로 측정하였다. 체중은 전자식 체중계(CAL-150, 한국)을 이용하여 신발을 벗고 가벼운 복장으로 측정하였다. 체질량지수(BMI)는 체중(kg)을 신장의 제곱(m2)으로 나누어 산출하였다. 허리둘레는 배꼽 높이에서 측정하고, 엉덩이둘레는 엉덩이 뒷부분 중 가장 돌출된 부분의 높이를 측정하였다. 허리·엉덩이둘레비(waist hip ratio: WHR)는 허리둘레를 엉덩이둘레로 나누어 산출하였고, 허리둘레·신장비(waist height ratio: WHtR)는 허리둘레를 신장으로 나누어 산출하였다.신체구성을 평가하기 위하여 체성분 분석기(X-scan PlusⅡ, Jawon Medical, 한국)를 이용해서 체지방률(%), 체지방량(㎏), 그리고 근육량(㎏)을 측정하였다.

2) 혈액 채취 및 성분 분석

모든 대상자는 실험 당일 12시간 공복 상태로 오전 8시 경에 검사실에 도착하여 30분간 안정을 취한 후 상완 주정맥(antecubital vein)에서 정맥혈을 채취하였다. 채취한 혈액은 항응고 처리된 튜브에 넣어 원심분리기를 이용하여 3,000 rpm으로 10분간 원심 분리한 후 세포성분을 제외한 혈장(plasma)만을 보관 튜브에 넣어 분석 시까지 –70℃의 냉동고에서 보관하였다. 혈액성분의 분석은 (주)G의료재단에 의뢰하였으며, 구체적인 분석 방법은 다음과 같다.

(1) 인슐린 저항성 지표

공복혈당 농도는 glucose oxidase 방법(Beckman glucose analyzer Ⅱ)으로 측정하였다. 공복인슐린은 전기화학 발광면역 분석법(electrochemiluminescence immunoassay: ECLIA)으로 면역 분석장비(Elecsys, Roche, 스위스)를 이용하여 분석하였다. Homeostasis model of assessment of insulin resistance(HOMA-IR)는 Matthews et al.(1985)이 제시한 다음의 공식을 이용하여 산출하였다

(2) 혈중 지질

TC는 cholesterol oxidase를 이용한 화학반응 원리로 분석하였고, TG는 자동분석기(ADVIA 1650, Bayer, 일본)를 이용해 효소 비색법(enzymatic calorimetric method)으로 분석하였다. HDL-C는 cholesterol oxidase를 이용하여 분석하였다. 수용성 염료의 침전제인 인텅스텐산과 마그네슘 양이온의 작용으로 LDL-C를 침전시켜 분석하였다. LDL-C는 효소면역검사법(enzyme immunoassay: EIA)을 이용하여 분석하였다.

3) 운동 프로그램

운동 집단의 대상자는 12주간 저항성운동 트레이닝 프로그램에 참여하였다. 이 프로그램은 ACSM(2009)이 제안한 Progression Model in Resistance Training Program에 기초하여 구성하였다. 이 운동 프로그램은 준비단계와 향상단계로 구분되어 있으며, 운동강도와 운동빈도를 점증적으로 증가시켰다. 운동강도는 운동자각도(ratings of perceived exertion: RPE)를 이용하여 설정하였다. 이 연구에서 실시한 저항성운동 프로그램의 구체적인 구성 내용은 <Table 2>에 제시된 바와 같다. 한편, 통제 집단의 대상자는 동일한 12주의 처치 기간 동안 평소의 신체활동습관, 식사습관, 그리고 수면패턴을 유지하도록 하였다. 이를 위하여 매주 1회씩 연락을 하여 지속적으로 확인하였다.

자료처리 방법

이 연구에서 얻은 결과를 SPSS PC+ for Windows (version 22.0)로 분석하였다. 각 집단에서 얻은 종속변인의 기술 통계량을 평균(mean)과 표준편차(standard deviation: SD)로 제시하였다. 두 집단 간, 그리고 두 검사 간 종속변인의 평균 차이를 검증하기 위하여 반복 이원변량분석(repeated two-way ANOVA)을 실시하였다. 주효과 또는 상호작용이 유의한 경우, 동일 집단 내 두 검사 간 차이를 분석하기 위하여 종속 t-검증(paired t-test)을 실시하였고, 동일 검사 내 사전검사와 사후검사 간 차이를 검증하기 위하여 독립 t-검증(independent t-test)을 실시하였다. 통계적 유의 수준(α)은 0.05로 설정하였다.

체격과 신체구성

체격과 관련된 변인 중 허리둘레, WHR, 그리고 WHtR에서 집단과 검사의 상호작용이 유의하게(p<.000) 나타났으며, 모든 변인에서 검사의 주효과가 유의하게 나타났다. 모든 변인에서 운동 집단의 사전·사후검사 간에 유의한 변화가 나타났다<Table 3>.

인슐린 저항성 지표와 관련된 변인 중 공복혈당과 HbA1c에서 검사의 주효과가 유의하게 나타났으나, 공복혈당만 두 집단에서 유의한(p<.01) 감소가 나타났다<Table 5>.

논 의

체격과 신체구성의 변화

이 연구에서는 20대 마른 비만 여성을 대상으로 12주간의 처치 후 체격의 변화를 알아보기 위하여 신장, 체중, 허리둘레, 엉덩이둘레, 허리·엉덩이둘레비(waist-hip ratio: WHR), 그리고 허리둘레·신장비(waist-height ratio: WHtR)를 측정하였고, 신체구성의 변화를 알아보기 위하여 체지방률, 체지방량, 그리고 제지방량을 측정하였다.

BMI는 비만 평가 지표로서 현재도 광범위하게 활용되고 있지만, BMI를 단독으로 이용하여 비만을 평가하는 것은 적합하지 못하다는 지적이 제기되어 왔다(Kyle et al., 2003). 실제로 Frankenfield et al.(2001)의 연구에서 BMI에 의하여 비만이라고 판정된 성인 남성 중 30%, 그리고 여성 중 46%만이 실제 비만인 것으로 나타나 BMI가 비만 평가 지표로서 적절치 못하다고 주장하였고, Yusuf et al.(2005)은 BMI의 감소가 곧 대사질환 위험의 감소를 의미하지는 않는다고 주장하면서 BMI의 효용성에 문제를 제기하였다.

이 연구에서 20대 마른 비만 여성을 대상으로 12주간 저항성운동을 처치한 결과, 운동 집단에서 허리둘레, 엉덩이둘레, WHR, 그리고 WHtR이 모두 유의하게 감소되었으며, 허리둘레, WHR, 그리고 WHtR에서 집단과 검사의 상호작용이 유의하게 나타났다. 이는 저항성운동이 비만 여성의 체형에 긍정적인 영향을 미쳤다고 보고한 So et al.(2008)와 Suk et al.(2009)의 연구 결과와 일치한다.

비만 평가 방법 중 허리둘레, 엉덩이둘레, WHR, 그리고 WHtR은 복부비만을 평가하는 중요한 지표이며, 특히 허리둘레와 WHR은 내장지방을 평가하는 가장 대표적인 간접측정방법이다(Yusuf et al., 2005). 마른 비만자는 정상인에 비해 내장지방이 많다고 보고된 바 있으며(Lee et al., 2015), Després & Lemieux(2006)는 복부비만이 대표적인 지방조직의 이상기능 지표인 동시에 대사증후군을 진단하는 지표로서 BMI보다 더 적합하다고 주장하였다. Welborn et al.(2003)은 심혈관계 위험 지표로서 WHR을 사용하는 것이 적합하며, 고혈압, 당뇨병, 그리고 이상지질혈증을 평가하는 가장 좋은 방법이라고 보고하였다. 그러나 특정 처치를 통하여 허리둘레와 엉덩이둘레가 동시에 감소하거나 증가되는 경우 WHR의 변화가 나타나지 않을 수 있기 때문에, 이와 같은 문제점을 보완하기 위한 지표로서 WHtR을 사용할 수 있다. WHtR은 신장이 변하지 않는 성인에게 있어서 효과적으로 사용될 수 있는 일종의 비만 지표로서, 심혈관계 질환 위험도와 밀접한 관계가 있는 것으로 보고되었다(Lee et al., 2008). 실제로 Hsieh & Muto(2004)은 BMI 25㎏·m⁻² 미만의 여성과 남성 모두 BMI 혹은 허리둘레를 단독 측정한 값보다 WHtR을 측정한 값이 관상동맥 위험도와 더 밀접한 상관관계를 가진다고 보고하였다. 또한 허리둘레와 WHtR은 과체중 어린이의 대사질환과 심혈관계 질환 위험요소 평가에 도움이 되는 것으로 보고되었으며(Savva et al., 2000; Maffeis et al., 2008), 성인의 경우 WHtR이 허리둘레나 BMI보다 심혈관계 질환 위험도를 평가하는데 더 적합하다고 알려졌다(Ashwell et al., 2012).

체지방률과 체지방량은 제2형 당뇨병 및 심혈관계 질환과 관련성이 크다고 알려져 왔다(Virtanen et al., 2005; Prado et al., 2012). 그러나 많은 연구에서 체지방률이 아닌 BMI만을 비만 판정의 지표로 이용하고 있어 마른 비만(normal weight obesity), 대사적으로 비만이지만 정상 체중(metabolically obese, but normal weight: MONW), 또는 대사적으로 정상이지만 비만(metabolically healthy, but obese: MHO) 등과 같이 BMI와 체지방률을 함께 이용하여 개인의 특성을 정의하는 용어가 생기게 되었다. 실제로 전술한 바와 같이 BMI가 30 ㎏·m-2 이상의 비만으로 분류되는 여성의 46%만이 실제 비만한 상태인 것으로 조사된 바 있어(Frankenfield et al., 2001) 체질량지수와 체지방률을 함께 감안하는 것이 바람직해 보인다. 체지방량을 측정하는 것은 정상, 과체중, 그리고 비만을 평가하는 가장 적합한 지표로 여겨지며, 이미 많은 연구에서 높은 체지방률이 제2형 당뇨병 및 심혈관계 질환의 위험도와 밀접한 관계가 있는 것으로 밝혀져 과도한 지방의 축적이 건강상의 문제를 야기할 수 있다고 보고되었다.

최근 좌식 생활과 과도한 칼로리 섭취에 따라 정상 체중인 국내 중년 여성 중 12.9%가 마른 비만자로 보고되었으며(Lee et al., 2011), 이와 같은 마른 비만자는 정상인에 비해 체지방률이 높고 신체활동으로 소비하는 에너지양이 현저히 낮은 것으로 조사되었다(Karelis et al., 2004). 좌식 생활은 고혈압 및 심혈관계 질환과 밀접한 관계가 있으며(Beunza et al., 2007; Warren et al., 2010), 근육이 위축되고 감소되는 근감소증을 유발하고 체지방률을 높여 결과적으로 완전한 비만한 상태를 유발하게 된다(Rey et al., 2008; Gianoudis et al., 2015).

단순히 체지방량을 감소시키는 것은 식이요법만으로도 충분하다. 그러나 식이요법은 칼로리 제한으로 제지방량과 근육량의 감소가 필연적으로 나타나므로(Jeong et al., 2005) 체지방량을 낮추고 제지방량을 유지하기 위해서는 유산소운동 혹은 저항성운동과 같이 신체활동량을 증가시키는 것이 가장 적절하다(Stiegler & Cunliffe, 2006). Lee et al.(2014)은 식이요법과 운동요법을 병행하여 10주간 처치한 결과 체지방량과 제지방량이 감소되었다고 보고하였고, Bann et al.(2014)은 여가시간에 신체활동을 하는 여성의 체지방량이 낮고 제지방량이 높았다고 보고하였다. 이 연구에서는 마른 비만 여성을 대상으로 12주간 저항성운동을 처치한 결과 운동 집단의 체중이 유의하게 감소되었으며, 체질량지수도 유의하게 감소되었다. 이에 더하여 통계적으로 유의한 변화는 나타나지 않았으나 운동 집단의 체지방률과 체지방량이 감소되는 경향을 보여 대부분의 선행 연구와 일치하였다. 체지방률의 감소폭이 유의하지 않은 것에 비하여 허리둘레, 엉덩이둘레, WHR, 그리고 WHtR이 유의하게 감소한 것은 이 연구에서 실시한 저항성운동 프로그램이 복부와 하체를 중심으로 이루어져 나타난 결과로 판단된다.

이상의 내용을 종합하면 이 연구에서 실시한 12주간의 저항성운동이 20대 마른 비만 여성의 체격과 신체구성을 개선하는 적합한 방법으로 판단된다. 다만, 저항성운동을 수행했음에도 불구하고 제지방량이 증가되지 않은 이유는 이 연구의 운동 프로그램이 근비대를 목표로 하는 프로그램이 아닌 전형적인 자기체중 운동(weight-bearing exercise) 트레이닝, 즉 근지구력 향상을 위한 프로그램이었기 때문이라고 사료된다.

인슐린 저항성 지표의 변화

인슐린은 췌장에서 분비되는 당조절 호르몬으로서, 혈당이 높아지면 포도당을 간과 근육 내 세포로 유입시켜 혈당을 낮추는 작용을 한다. 그러나 고지방식사, 운동 부족, 그리고 비만과 같은 이유로 인하여 세포 표면의 인슐린 수용체가 부족해지거나 인슐린에 항체가 생성되어 인슐린이 제대로 기능하지 못하는 현상이 생기는데 이를 인슐린 저항성(insulin resistance)이라고 한다. 인슐린 저항성이 높은 경우 인슐린이 충분히 분비됨에도 불구하고 혈당이 정상 수준으로 떨어지지 않아 높아진 혈당이 계속하여 인슐린 분비를 촉진하는 악순환이 반복된다(Bloomgarden, 1998). 인슐린 저항성이 악화될수록 각종 질병으로 인한 사망률이 높아지며(Mohan et al., 2014), 높아진 인슐린 저항성은 제2형 당뇨병, 고혈압, 그리고 심혈관계 질환의 위험도와 밀접한 관계를 보인다고 보고되었다(Lamounier et al., 2006).

다수의 연구에서 인슐린 저항성을 개선시키는 방법으로 식이요법과 운동요법을 제시하고 있으며, 이와 관련된 연구가 활발하게 수행되어 왔다(Tamura et al., 2005; Solomon et al., 2008). Ross et al.(2004)은 비만 여성을 대상으로 14주간 유산소운동을 처치한 결과 인슐린 저항성 지표가 개선되었다고 보고하였고, Ibáñez et al.(2010)은 중년의 비만 여성을 대상으로 16주간 주2회 저항성운동과 식이요법을 실시한 결과 역시 인슐린 저항성이 개선되었다고 보고하였다. Huh(2012)는 체지방률이 30% 이상인 여고생을 대상으로 영양교육 및 복합운동을 처치한 결과 공복혈당, 공복인슐린, 그리고 HOMA-IR이 개선되었다고 보고하였으며, Leon et al.(2014)은 비만 여성을 대상으로 6개월간 운동을 포함한 체중감소 프로그램을 처치한 결과 체지방이 감소한 여성의 인슐린 민감성(insulin sensitivity)이 개선되었다고 보고하였다. 이와 같은 연구 결과를 통하여 운동요법이 인슐린 저항성의 개선에 긍정적인 영향을 미치는 것을 알 수 있다.

이 연구에서는 12주간의 저항성운동을 처치한 결과 공복혈당을 제외한 모든 변인에서 유의한 변화가 나타나지 않아 관련 선행 연구와 다소 다른 상이한 결과가 도출되었다. 그러나 이 연구 결과를 지지하는 보고로 Ha & So(2012)는 20대 여대생을 대상으로 복합운동을 실시한 결과 공복혈당이 유의하게 변하지 않았고, Kim et al.(2013)도 24주간 중년 여성을 대상으로 중강도 걷기운동을 실시한 결과 공복혈당, 공복인슐린, 그리고 HOMA-IR의 유의한 변화가 없었다고 보고하였다. Poehlman et al.(2000)은 정상 체중의 젊은 여성을 대상으로 유산소운동과 저항성운동을 처치한 결과 유산소운동 집단에서는 인슐린 민감성이 향상된 반면, 저항성운동 집단에서는 인슐린 저항성 지표의 유의한 변화가 없었다고 보고하였다. 한편, Sigal et al.(2007)은 제2형 당뇨병을 앓고 있는 성인을 대상으로 주3회씩 22주간 유산소운동, 저항성운동, 그리고 복합운동을 처치한 결과 유산소운동 집단, 저항성운동 집단, 복합처치 집단 모두 인슐린 저항성이 개선되었으나 복합처치 집단에서 가장 큰 변화가 있었다고 보고한 바 있다.

전술한 선행 연구의 결과를 종합해 보면, 다수의 연구에서 운동을 통하여 인슐린 저항성이 개선되었지만, 일부 연구에서는 이와 다른 결과가 나타났다. 인슐린 저항성이 개선된 것으로 보고한 대부분의 선행 연구의 대상자는 사전검사 시점에서 인슐린 저항성 지표가 악화되어 있어 운동 처치를 통한 변화의 여지가 컸던 것으로 보인다. 이 연구의 대상자는 사전검사 시점에서 인슐린 저항성 지표 수준이 정상 범위에 있었기 때문에 통계적으로 유의한 변화를 끌어내는데 한계가 있었을 것으로 사료된다. 아울러 이 연구에서 저항성운동만을 처치했던 점도 인슐린 저항성 지표에 미치는 영향이 적었던 다른 하나의 이유가 될 것으로 판단된다. 향후 인슐린 저항성 상태가 악화되어 있는 높은 연령층의 마른 비만자를 대상으로 유산소운동과 저항성운동을 포함한 복합운동 처치를 실시한다면 인슐린 저항성의 유의한 개선을 기대할 수 있을 것이다.

혈중 지질의 변화

과도한 지방조직의 축적은 체지방량을 늘리고 비만 상태를 유발하며, 혈액 내 지질 수준이 높아지는 현상을 초래한다. 혈중 지질은 이상지질혈증과 심혈관질환의 발병위험과 연관이 있으며, 이와 같은 질병의 예방을 위하여 혈중 지질 수준을 개선해야 한다는 연구들이 보고되고 있다(Klop et al., 2013). 또한 혈중 지질 수준이 높을수록 인슐린 저항성을 악화시켜 제2형 당뇨병 발병 위험 수준을 높인다는 보고도 있었다(Haffner, 1998; Kolluri et al., 2009; Gholap et al., 2011). 따라서 혈중 지질 수준을 개선시켜 이와 같은 대사질환을 예방해야 하며, 대표적인 개선 방법으로 식이요법과 운동요법이 제시되어 왔다(Chudyk & Petrella, 2011; Rognmo et al., 2012; Rees et al., 2013).

혈중 지질 수준을 개선시키는 대표적인 방법 중 하나인 운동요법은 과거부터 최근까지 그 효과에 대한 연구가 지속적으로 수행되어 왔으며, 그 효과에 대하여 연구마다 다른 결과를 도출해 왔다. Ha & So(2012)는 20대 여대생을 대상으로 12주간 주3회 복합운동을 실시한 결과 TG를 비롯한 TC와 LDL-C가 유의하게 변화되지 않았고, HDL-C가 당초 예상과 달리 유의하게 감소되었다고 보고하였다. 또한 Cho(2009)는 중년 여성을 대상으로 8주간 저칼로리 식이와 유산소운동을 처치한 결과 TC가 유의하게 변화되지 않았다고 보고하였다. Hasbum et al.(2006)은 성인 여성을 대상으로 2개월간 주3회 중강도의 유산소운동을 처치한 결과 TC, TG, 그리고 LDL-C의 유의한 변화가 없었다고 보고하였다. Ho et al.(2012)은 과체중과 비만자를 대상으로 12주간 주5회 유산소운동, 저항성운동, 그리고 복합운동을 실시한 결과 모든 운동 집단에서 TG, TC, LDL-C, 그리고 HDL-C의 유의한 변화가 없었다고 보고하였다. 반면, Kang & Kim(2002)은 비만 여성을 대상으로 8주간 주3회 복합운동을 처치한 결과 TG가 감소되고 HDL-C가 증가되었다고 보고하였다. Cho et al.(2011)은 체지방률 30% 이상의 중년 여성을 대상으로 12주간 주3회 요가와 저항성운동을 처치한 결과 TG와 LDL-C가 유의하게 감소되고 HDL-C가 증가되었으나 TC의 유의한 변화는 없었다고 보고하였다. Pattyn et al.(2013)은 건강한 성인을 대상으로 유산소운동을 처치한 결과 HDL-C가 증가된 반면 TG가 유의하게 변화되지 않았다고 보고하였다.

이상에서 살펴본 바와 같이, 운동 트레이닝에 따른 혈중 지질관련 변인의 변화는 연구마다 상이하게 나타났다. 그 이유는 혈중 지질이 운동 트레이닝의 유형, 강도, 그리고 기간의 영향을 받으며, 그 이외에도 식습관을 포함한 다양한 생활습관의 영향을 많이 받기 때문이라고 판단된다. 이 연구에서는 운동 집단의 TG가 통제 집단에 비하여 더 크게 감소되는 경향을 보였으나 통계적으로 유의한 수준의 변화는 나타나지 않았으며, TC, LDL-C, 그리고 HDL-C은 운동 집단과 통제 집단 모두에서 유의한 변화가 나타나지 않았다. 많은 선행 연구에서 마른 비만 여성의 혈중 지질 특성 및 심혈관계 질환 위험도가 일반 비만자와 유사하다고 하였으나, 이 연구의 대상자들은 높은 체지방률을 보임에도 불구하고 혈중 지질이 정상 범위에 있었기 때문에 변화의 여지가 적었던 것으로 판단된다. 따라서 인슐린 저항성의 경우와 마찬가지로 향후 대사적으로 더욱 악화되어 있는 대상자를 선정하여 저항성운동 처치를 한다면 혈중 지질의 유의한 변화를 끌어낼 수 있을 것으로 판단된다.

결 론

이 연구의 목적은 12주간의 저항성운동이 20대 마른 비만 여성의 체격, 신체구성, 인슐린 저항성 지표, 그리고 혈중 지질에 미치는 영향을 규명하는 것이었다. 이 연구에서 얻은 결과를 요약하여 제시하면 다음과 같다.

1) 운동 집단의 체중, BMI, 허리둘레, 엉덩이둘레, WHR, 그리고 WHtR이 유의하게 감소되었다. 2) 신체구성과 관련된 변인은 두 집단 모두에서 유의하게 변화되지 않았으나, 운동 집단의 체지방량이 통제 집단에 비하여 더 크게 감소된 것으로 나타났다. 3) 인슐린 저항성 지표 중 공복혈당은 두 집단 모두에서 유의하게 감소되었으며, 다른 변인은 두 집단 모두에서 유의한 변화가 나타나지 않았다. 4) 혈중 지질과 관련된 변인은 운동 집단과 통제 집단 모두에서 유의한 변화가 나타나지 않았다.

이상의 결과를 종합해 보면, 12주간의 저항성운동이 20대 마른 비만 여성의 체격의 개선에 도움을 주고 신체구성에 긍정적 영향을 미쳤으나, 인슐린 저항성과 혈중 지질에는 유의한 영향을 미치지 못한 것으로 나타났다. 따라서 향후 신체기능이 가장 우수한 20대가 아닌 중년 이상에서 마른 비만자를 선정하여 후속 연구를 한다면 인슐린 저항성과 혈중 지질 등 대사 지표에 대한 저항성운동의 효과를 보다 명확하게 파악할 수 있을 것으로 기대되는 바, 관련 후속 연구가 요청된다.
