Abstract

우리는 대한민국 대학스포츠의 부상실태를 2015년 한 시즌동안 (2015/01/01 – 2015/12/16) 조사하였다. 시즌 전, 시즌 중, 그리고 시즌 후 동안 모든 훈련, 연습경기, 그리고 공식경기에 참여한 6개 종목(배드민턴, 야구, 농구, 럭비, 태권도, 그리고 배구)의 학생운동선수들(n=167명)은 부상조사시스템(부상률, 부상 부위, 부상 형태, 그리고 부상 기전: 각 종목의 연구원이 기록)을 통하여 매일 추적하여 기록하였다. 수집된 데이터는 1,000 선수노출(Athlete-Exposure: AE)과 1,000 시간노출(Time-Exposure: TE)에 대한 부상률을 95% 신뢰도구간(Confidence intervals: CI)과 함께 계산되었다. 총 961건의 부상이 기록되었으며, 19.29/1,000 AE (95% CI=18.07, 20.51)과 0.16/1,000 TE (95% CI=0.15, 0.17)의 부상률을 보였다. 주로 일어난 부상 부위는 무릎(17.5%, 168명), 허벅지(16.1%, 155명), 발목(12.1%, 116명), 그리고 손가락(8.4%, 81명)이었다. 부상 형태로는 타박상(40.1%, 385명), 인대염좌(21.1%, 203명), 그리고 피부조직부상(13.53%, 130명)이었다. 주된 부상 기전으로는 상대선수와의 접촉(44.6%, 429명), 움직이는 물체와의 접촉(12.7%, 122명), 과 사용/비 사고(11.9%, 114명), 그리고 움직이지 않는 물체와의 접촉(9.4%, 90명)이었다.

Trans Abstract

The purpose of this study was to describe the epidemiology of Korean collegiate injuries during 2015. Collegiate student-athletes(n=167) in six sports (badminton, baseball, basketball, rugby, taekwondo, and volleyball) participating in all practices and games during pre-season, in-season, and post-season were tracked via the injury surveillance system(rate, location, type, and cause of musculoskeletal injury). Injury rate for 1000 athlete-exposure(AE) and 1000 time-exposure(TE) were calculated with 95% confidence intervals(CI). We captured a total of 961 injuries, a rate of 19.29/1000 AE(95% CI=18.07, 20.51) and 0.16/1000 TE(95% CI=0.15, 0.17). Commonly injured body locations were knee(17.5%, n=168), thigh(16.1%, n=155), ankle(12.1%, n=116), and finger(8.4%, n=81). Injuries were mostly diagnosed as contusion(40.1%, n=385), ligament sprain(21.1%, n=203), and laceration/abrasion/skin injury(13.5%, n=130). Common causes were contact with another athlete(44.6%, n=429), contact with moving object (12.7%, n=122), overuse/atraumatic(11.9%, n=114), and contact with non-yielding object(9.4%, n=90).

서 론

스포츠는 경쟁이 요구되며 이에 따라 부상은 피할 수 없다. 스포츠 참가로 인한 부상의 빈도를 줄일 수 있는 방법으로 부상실태조사가 있다. 예를 들어, 종목, 나이, 성별, 경기/훈련, 그리고 시즌/비시즌 별 부상패턴과 종류를 파악해 볼 수 있다. 그 데이터를 기초로 특정 부위의 근력을 강화하거나 유연성을 높여줌으로서 부상을 예방 하거나, 특별한 기전이 파악될 경우 규칙을 개정할 수도 있다. 스포츠 부상실태조사는 미국 미네소타 대학의 헬스서비스에서 처음 시작하였으며, 1955-1956년부터 1964-1965년까지 9년간 26,000건의 부상을 보고했다(Kraus & Gullen, 1969).

국제대회에서 선수부상에 대한 조사 분석은 활발히 진행되고 있다. 2008년 베이징 올림픽대회 동안 72.5%의 부상은 시합 중에 발생하였고, 그 중 33%의 부상은 상대 선수와의 접촉에 의한 것, 20%는 비접촉, 그리고 22%는 과 사용으로 인한 부상이라고 보고하였다(Junge et al., 2008). 2012년 런던 올림픽대회에서는 1,361건의 부상과 758건의 질병을 기록하였고, 대회 중 가장 부상위험빈도가 높은 종목은 태권도, 축구, Bike Motorcross, 핸드볼, 산악자전거, 육상, 역도, 하키, 그리고 배드민턴 순이었고, 가장 낮은 종목은 양궁, 카누, 사이클, 조정, 사격, 그리고 곡마 순이라고 보고하였다(Engebretsen et al., 2013). 또한, 2010년 밴쿠버 동계 올림픽대회에서 2,567명(남자 1,522명, 여자 1,045명)이 출전하여 대회 기간 동안 287건의 부상과 185건의 질병이 집계 되었으며, 선수 1,000명 당 111.8건의 부상과 72.1건의 질병의 부상률이 나타났다고 보고하였다(Engebretsen et al., 2010).

미국대학스포츠협회(National Collegiate Athletic Association: NCAA)는 학생운동선수에 대한 부상 실태를 활발히 보고하고 있다. 미국대학스포츠협회는 1988년부터 16개 스포츠 종목(남자야구, 여자체조, 남자아이스하키, 남자라크로스, 여자라크로스, 남자축구, 여자축구, 여자소프트볼, 여자배구, 그리고 남자레슬링)으로부터 훈련 및 경기 참가로 인한 부상 데이터를 수집해 왔으며, 2000년도부터 한 종목(여자아이스하키)을 더 포함시켰다(Hootman et al., 2007). 그 결과로, 1997년부터 2000년까지의 집계조사 결과, 여자라크로스선수 13.9%, 여자축구 11.4%, 남자아이스하키 10.3%, 남자라크로스 10.1%, 미식축구 8.8%, 여자농구 8.5%, 필드하키 7.2%, 남자축구 7.0%, 그리고 레슬링 6.6% 순으로 뇌진탕을 경험하는 것으로 조사되었다(Covassin et al., 2003). 또한, 1989년부터 2004년까지 조사한 한 연구에서는 총 4,080건의 어깨관절의 불안정성이 기록되었으며, 이 결과는 1,000회의 선수 노출(Athlete-Exposure: AE)을 기준으로 했을 때 0.1건의 부상률을 나타낸다고 보고하였다(Owens et al., 2009).

우리나라에서도 선수들을 대상으로 한 부상조사는 꾸준히 이루어지고 있다. 1988년 서울 장애인 올림픽 국가대표운동선수 125명을 대상으로 한 설문조사에서 운동 상해의 원인으로 선수과실(52.8%), 과도한 훈련(22.4%), 그리고 보조기구문제(15.2%) 순으로 응답하였다. 운동 상해는 전체손상 216건의 손상 부위 중 손 46건(21.3%), 발 42건(19.4%), 그리고 어깨관절부 39건(18.1%)으로 나타났다고 보고하였다(Choi et al., 1989). 1984년 로스앤젤레스 올림픽 대회부터 1996년 애틀랜타 올림픽 대회까지 배구대표 선수들은 허리 16건(30.2%), 무릎 15건(28.3%), 그리고 어깨관절에 12건(22.6%)의 부상을 경험하였으며, 부상 시기는 훈련 중 47건(75.8%), 경기 중 15건(24.2%)으로 나타났다고 보고하였다(Kim, 1997). 이밖에 고등학교 태권도 선수(Jung, 2010), 전국체전에 참가한 남자체조 선수(Kim et al., 2012), 그리고 배드민턴 국가대표 선수(Kim et al., 2013), 그리고 런던 파랄림픽 대회 참가 선수(Lee & Choi, 2014)를 대상으로 설문조사하여 부상률을 보고한 국내 연구들이 있다.

살펴본 바와 같이, 국내의 스포츠 부상조사는 대부분 시합이나 훈련이 끝나고 1회성의 설문에 의존하고 있어, 실시간으로 주기적으로 부상실태를 관찰하고 기록하는 시스템은 구축되어 있지 않다. 설문지 이용은 지나간 과거를 회상하여 기록되므로 병원진료가 필요했던 큰 부상만 기록되기 쉽고, 대회기간 동안의 부상을 주로 조사함으로 시즌 전(Pre-season) 그리고 후(Post-season)의 부상파악이 어렵다. 무엇보다도 몇 명이 몇 분 동안 연습과 경기에 참가했는지 파악하기 힘들어 선수노출(AE) 이나 시간노출(Time-Exposure: TE)에 따른 부상빈도를 파악하는 것이 불가능하다.본 연구는 2015년 일 년 동안 대학 스포츠 현장에서 발생한 부상을 매일 추적하여 훈련과 연습경기, 그리고 공식경기의 시간과 참여한 학생운동선수 숫자를 시즌 전, 시즌, 그리고 시즌 후로 구분하여 해당기간에 수집된 근 골격계 부상의 부위, 형태, 그리고 기전에 대해 기록하였다. 따라서 본 연구의 데이터 수집과 처리방법은 향후 국내에서 진행될 부상기록시스템의 빅데이터 구축을 위한 파일럿 데이터로 활용될 것이며, 그 결과는 부상관리/예방 프로그램의 개발에 기초 자료를 제공할 것이다.

연구방법

연구 참여자

국내 한 대학교 내 6개 종목(배드민턴 10명, 야구 36명, 농구 17명, 럭비 37명, 태권도 51명, 그리고 배구 16명)의 학생운동선수 167명(age: 20.8 ± 1.4years; height: 180.6 ± 8.5cm; mass: 79.6 ± 14.4kg; athletic career: 8.5 ± 2.6years)이 본 연구에 참여하였다.

자료 수집

2015년 1월 1일 부터 12월 16일 까지 국내 한 대학교 내, 외에서의 훈련(체력훈련, 기술훈련, 그리고 연습경기)과 공식경기(홈과 원정경기)에 참여한 학생선수 숫자와 시간, 그리고 기간 동안 일어나는 모든 근 골격계 부상(부위, 형태, 그리고 기전)을 실시간으로 (훈련과 공식경기 직 후) 스프레드 시트 소프트웨어(Excel, Microsoft Corp., Redmond, WA, USA)에 기록하였다. 각 운동부에 소속된 연구원 6명은 일관된 데이터 수집을 위해 아래 용어 정의와 기록에 대한 교육을 받았다. 6명의 연구원은 매달 1일에 이전 달 데이터를 총괄 연구자에게 전달하였으며, 총괄 연구자는 모든 데이터를 취합하여 분석하였다.

용어 정의

근 골격계 부상은 다음과 같이 정의하였다: “공식적인 팀 훈련, 연습경기, 혹은 공식경기 중 일어난, 급성 혹은 만성적인 근 골격계의 불편(complaint)”으로 인해, 훈련, 연습경기, 혹은 공식경기로부터 최소 한번의 이벤트에 제외되는 것.”(Kerr et al., 2015).

선수노출(AE)은 한 명의 선수가 정상적인 훈련, 연습경기, 혹은 공식경기에 참가하는 빈도수로, 시간에 관계없이 부상의 가능성에 노출되어 있는 상태를 말하며, 시간노출(TE)은 한 명의 선수가 정상적인 훈련, 연습경기, 혹은 공식경기에서 실제로 뛰는 시간만 분 단위로 포함되었다(Rechel et al., 2008). 예를 들어, 한명의 선수가 오전에 웨이트트레이닝 두 시간 오후에 전술훈련 두 시간을 참가하였다면, 선수노출은 2, 시간노출은 240분으로 기록되었다. 이 선수가 속한 종목의 선수가 20명 이라면, 선수노출은 40, 시간노출은 4,800분으로 기록되었다.

시즌 전, 시즌, 그리고 시즌 후의 구분은 종목에 따라 달리 정의되었다. 주로 시즌은 해당년도 첫 공식경기(리그 혹은 토너먼트) 시작일로 부터 마지막 공식경기까지로 보는 것이 일반적이다(Dick & Marshall, 2007).따라서 본 연구에서도, 시즌은 해당 종목의 처음과 마지막 공식경기 날을 기준으로 하였다<Table 1>.

자료 분석

선수노출 1,000명 당 부상빈도와 시간노출 1,000분 당 부상빈도, 그리고 각 값의 95% confidence intervals (CI)를 계산하기 위해 아래 공식을 사용하였다(Kerr et al., 2015).

부상 부위는 얼굴(눈, 귀, 코 포함), 머리, 목/경추, 흉추/윗 등, 흉골/갈비뼈, 요추/허리, 복부, 골반/천추/둔부, 어깨/쇄골, 윗 팔, 팔꿈치, 앞 팔, 손목, 손, 손가락, 엉덩이부위, 사타구니, 허벅지, 무릎, 아랫하체, 아킬레스건, 발목, 그리고 발/발가락으로 분류하였다. 부상 형태로는 뇌진탕, 골절(부상, 스트레스, 다른 뼈부상), 인대염좌(탈구, 아탈구, 인대파열), 근육/건 염좌(근육/건 파열), 반월상연골판/연골, 타박상/울혈/멍, 건염, 관절염/활액막염/활액낭염, 충돌, 열상/찰과상/피부조직손상, 치아 부상, 근육경련, 그리고 기타로 분류하였다. 열상/찰과상/피부조직손상과 근육경련을 제외한 나머지 부상은 병원을 방문하여 진단을 받은 후 그 결과를 기록하였다. 부상 기전은 과 사용(점진적 발생), 과 사용(급진적 발생), 비접촉부상, 과거부상재발, 상대선수와 접촉, 움직이는 물체와 접촉, 고정되어 있는 물체와 접촉, 반칙, 경기장 혹은 플레이상태, 장비오류, 그리고 기타로 분류하였다(Orchard & Seward, 2002; Brooks et al., 2005; Agel et al., 2007; Kerr et al., 2015).

결 과

부상률

전체 부상률은 <Table 2>에 제시하였다. 총 961건(시즌 전 298건, 31%; 시즌 중 615건, 64%; 시즌 후 48건, 5%)의 부상이 기록되었다. 시즌의 구분과 상관없이, 2015년 훈련 중 일어난 부상은 총 536건(55.8%), 연습경기에서 207건(21.5%), 공식경기에서 218건(22.7%)이 집계 되었다. 월별로는 1월과 3월에 175건으로 가장 높았고, 8월 98건, 그리고 4월 92건 순이었다.

총 선수노출(AE)은 49,829명으로, 선수 한 명이 훈련과 경기에 1,000번 참여 하였을 때 약 20번의 부상(19.3/1,000AE; 95% CI=18.07, 20.51)을 경험하는 것으로 나타났다. 총 시간노출(TE)은 6,106,916분으로, 훈련과 경기에 참여한 1,000분 당 0.16번의 부상(0.2/1,000TE; 95% CI=0.15, 0.17)을 경험하는 것으로 분석되었다.

선수노출에 따른 시즌 별 부상률을 살펴보면, 시즌 전(14,726AE)과 시즌 중(30,766AE)이 약 20회로 동일하며, 이는 시즌 후 부상률(4,337AE)보다 두 배 정도 높은 수치이다<Table 2>. 시간 노출에 따른 시즌 별 부상률을 살펴보면, 시즌 전(2,022,585TE)과 시즌 중(3,481,661TE)이 각각 0.2회와 0.2회로 유사하며, 시즌 후 부상률(602,670TE) 0.1회보다 두 배 정도 높은 것으로 나타났다.

부상률은 시즌 중 공식경기에서 가장 높았으며(143.6/1,000AE; 1.5/1,000TE), 시즌 후 훈련(8.2/1,000AE; 0.1/1,000TE)이 가장 낮았다.

부상 부위

전체 부상 부위는 <Table 3>에 제시하였다. 무릎(168건, 17.5%)과 허벅지(155건, 16.1%) 부상이 가장 많았으며, 발목(116건, 12.1%), 손가락(81건, 8.4%), 아랫하체(75건, 7.8%), 발/발가락(69건, 7.2%), 그리고 어깨/쇄골(53건, 5.5%) 순으로 빈번하게 부상을 입은 것으로 나타났다.

부상 형태

전체 부상 형태는 <Table 4>에 제시하였다. 타박상(385건, 40.1%)이 가장 많았으며, 인대염좌(203건, 21.1%), 피부조직부상(130건, 13.5%), 근/건 염좌(58건, 6.0%), 만성건염(31건, 3.2%), 골절(18건, 1.9%), 뇌진탕(14건, 1.5%), 충돌(14건, 1.5%), 그리고 관절염/활액막염/활액낭염(9건, 0.9%) 순으로 조사 되었다.

부상 기전

전체 부상 기전은 <Table 5>에 제시하였다. 상대 선수와의 접촉에 의해 일어난 부상은 429건으로 전체의 44.6%의 비중을 차지하였고, 그 다음 주요 기전으로는 움직이는 물체와 접촉하여 일어난 부상(122건, 12.7%)이나 급진적으로 진행된 과 사용으로 인한 부상(114건, 11.9%) 이었다, 10% 미만으로 나타난 기전으로는, 고정되어 있는 물체와 접촉하여 일어난 부상(90건, 9.4%), 비접촉성 급성부상(80건, 8.3%), 점진적으로 진행된 과 사용으로 인한 부상(78건, 8.1%), 경기장이나 플레이 상태(38건, 4.0%), 그리고 과거 부상의 재발(7건, 0.7%)이 있었다.

논 의

전체 부상률

시즌 중(In-season) 공식경기 동안 일어난 부상률(143.6/1,000AE, 1.5/1,000TE)은 같은 기간 훈련(10.3/1,000AE, 0.1/1,000TE) 혹은 연습경기(72.1/1,000AE, 0.4/1,000TE) 보다 훨씬 높았다. 1988년부터 2004년, 총 16년 동안 미국대학스포츠협회에서 조사한 15개 종목의 부상분석 데이터에서(Hootman, 2007), 공식경기 중 부상률(13.8/1,000AE)이 훈련 중 부상률(4.0/1,000AE)보다 높아 본 연구와 유사한 결과를 보고하였다. 이 외 많은 부상조사분석 연구들(Agel et al, 2007; Dick et al., 2007; Fuller et al., 2012; Hootman et al., 2007)이 시즌 중 공식경기의 부상률이 훈련이나 연습경기 때 보다 높다고 보고하였다. 훈련이나 연습경기는 공식경기에 비해 경쟁이 덜하고 경기의 전략이나 전술위주로 움직이기 때문에 부상률이 낮다. 예를 들어, 럭비는 경기 중 접촉이 가장 심한 종목 중 하나이고, 훈련과 공식경기간의 상대와 접촉하는 강도와 빈도에 상당한 차이가 있기 때문에 공식경기 중 부상이 훈련기간에 비해 월등히 많은 것으로 판단 된다. 태권도 또한 상대를 타격하여 점수를 내는 접촉 종목이기 때문에, 훈련에는 부상을 최소할 수 있는 프로그램(강도)으로 구성되고 공식경기 때 실제로 승부를 내기위해 접촉의 강도와 빈도가 늘어나 부상이 많은 것으로 판단된다.

전체 부상 부위

가장 많이 다친 세 부위는 무릎(168건, 17.5%), 허벅지(155건, 16.1%), 그리고 발목(116건, 12.1%) 이다. 본 연구의 6개 종목은 체중을 지탱한 상태에서 빠르게 움직여야 하는 동작이 필수적이라고 보았을 때 하체부상이 다른 부위보다 상대적으로 많은 것은 당연한 결과라고 여겨진다. 하체의 주요관절인 무릎(17.5%), 발목(12.1%), 그리고 발(8.2%)의 부상은 353건으로 전체 부상의 37.8%였고, 상체부상 중 주요관절인 허리(36건, 3.8%), 어깨(53건, 5.5%), 그리고 팔꿈치(32건, 3.3%)의 부상은 전체 부상의 12.6%를 차지하여, 하체에서 일어나는 주요부상이 상체에 비해 3.6배 많았다.

위 설명과 같이 타박상(385건, 40.1%)은 전체 6개 종목에서 일어나는 부상으로 가장 많았는데, 비접촉 종목인 배드민턴에서는 타박상으로 인한 부상은 없었고, 배구는 7건 있었다. 배구 역시 상대방과 접촉하지 않는 종목이지만, 공의 강한 타격에 의한 것으로 타박상이 발생하는 것으로 판단 된다. 접촉 종목인 태권도는 168건(전체의 43.6%)으로 6개 종목 중 가장 많은 타박상이 기록되었는데, 경기에서 이기기 위해서 상대를 타격하여 점수를 얻어야 하기 때문에 상대를 공격하는 동안이나 상대 공격을 막는 과정에서 타박상은 피할 수 없는 부상 일 것이다. 선행연구의 태권도(Covarrubias et al., 2015)와 럭비(Fuller et al., 2012) 종목의 통계를 보면, 타박상은 미국 대학생 태권도 시합에서 41.7%(총 115건 중 48건)와 2011년 럭비월드컵에서 전체부상의 13.7%를 차지하여 접촉이 있는 종목에서 타박상의 부상률이 가장 높은 것을 알 수 있었다. 타박상을 예방하는 방법은 종목의 특성과 룰에 따라 제한적이므로 예방법 보다는 부상 후 시기적으로 정확하고 적절한 처치를 잘 하는 것이 중요하다고 판단된다.

럭비(168건)와 태권도(111건) 종목의 타박상은 전체 부상의 29.1%를 차지하였고, 그 결과로 인대염좌(203건, 21.1%)를 두 번째 부상이 되게 하였다. 이는 인대염좌가 가장 많은 부상 형태라고 보고했던 선행연구(미국대학스포츠협회: Hootman et al., 2007)와 다른 결과를 보여주고 있다.

골절은 총 18건이 발생하였으며, 태권도(10건), 농구(2건), 럭비(2건), 배구(2건), 배드민턴(1건), 그리고 야구(1건)로 나타났으며, 접촉이 가장 많은 태권도가 다른 종목에 비해 5배 이상 골절 빈도가 높았다. 뇌진탕은 총 14건 발생하였으며, 럭비(11건)가 가장 많았고, 태권도, 배드민턴, 그리고 배구는 뇌진탕 부상이 없었다.

전체 부상 기전

6개 종목에서 일어난 총 부상(961건)에서 상대 선수와 접촉에 의해 일어난 부상(429건, 44.6%)이 가장 많았는데, 그 중 태권도(186건)와 럭비(181건)가 367건으로 85.6%를 기록하여 종목의 특성에 따른 부상패턴을 잘 보여주고 있다. 다음으로 움직이는 물체와의 접촉으로 인해 일어난 부상(122건)은 구기종목인 배드민턴, 야구, 농구, 럭비, 그리고 배구에서 셔틀콕이나 다른 4개 종목에서 사용하는 공과의 접촉하여 일어난 부상으로 추측된다. 과 사용으로 인한 부상(192건, 20.0%)은 접촉부상 다음으로 많았으며, 선수 경력이 평균 8.5년으로 장시간의 훈련과 경기의 반복에 대한 결과로 보여진다. 현재 국내 대부분의 중 고등학교 시스템에서 선수의 부상을 관리하는 체계는 거의 존재하지 않는 실정이며, 중·고등학교의 학생운동선수의 건강한 선수생활을 위해서는 부상조사가 반드시 이루어져야 하고 이를 바탕으로 한 부상예방시스템의 구축이 필요할 것이다.

배드민턴

배드민턴은 총 16건의 부상이 있었으며, 이 중 접촉에 의한 부상은 2건으로, 이는 셔틀콕에 맞으면서 일어난 부상 1건과 훈련 중 다른 선수와의 접촉에 의한 부상 1건 이었다. 만성부상은 10건으로 급성부상(5건)보다 두 배 많았는데, 접촉이나 충돌로 인해 생기는 사고의 가능성이 다른 접촉성 스포츠에 비해 현저히 낮기 때문에 상대적으로 많은 만성부상이 기록된 것으로 보인다. 부상 형태로 인대염좌(4건, 25%), 근육통(4건, 25%), 그리고 근/건 염좌(3건, 20%) 순으로 집계되었다. 2015년 뉴질랜드의 배드민턴 종목에서 하지 부상에 대한 조사가 있었는데, 95%의 부상은 연부조직부상(타박상, 건/인대염좌)이었고, 접촉에 의한 부상은 없었다(Reeves et al., 2015).

야구

야구는 총 270건의 부상이 있었으며, 무릎 부상(37건, 13.7%), 허벅지(30건, 11.1%), 손(30건, 11.1%), 손가락(27건, 10.0%), 발목(23건, 8.5%), 아랫하체(23건, 8.5%), 앞 팔(17건, 6.3%), 팔꿈치(16건, 5.9%), 발/발가락(14건, 5.2%), 그리고 어깨(11건, 4.1%)으로 다양하게 조사되었다. 부위별로 살펴보면, 상체부상이(142건, 52.6%) 하체부상(128건, 47.4%)보다 많았다. 손과 손가락 부상이 57건으로 전체부상의 21.1%를 차지 하였는데, 이는 야구선수들이 공을 던지는 과정 중 가속구간(acceleration phase)에서 공을 손가락으로 강하게 긁어낼 때 손톱이 부러지거나 찢어지는 부상이 많았기 때문으로 추측된다. 이러한 손 부상을 효과적으로 예방하기 위해서 알맞은 예방법(예: 매니큐어와 송진의 사용)을 개발해 보급할 필요가 있다. 부상의 형태를 보면, 찰과상이나 열상 등의 피부조직 손상(95건, 35.2%)이 있었고, 타박상(83건, 30.7%), 인대염좌(32건, 11.9%), 그리고 근육통(25건, 9.3%) 순으로 조사되었다. 미국대학스포츠협회에서 1988년부터 2004년 까지 대학야구의 부상을 조사한 연구에 따르면, 본 연구결과와 마찬가지로 공식경기 중 상체부상(44.6%)이 하체부상(35.2%)보다 많은 경향을 보였다(Dick et al., 2007). 특이한 점으로, 본 연구에서는 머리부상이 2회 기록되었는데, 17년간 148건의 머리부상을 보고했던 선행연구(Dick et al., 2007)와 비교하면 1년 동안의 기록이라 하더라도 그 빈도수가 훨씬 적은 것으로 나타났다. 이 점은 아마도 선수들이 머리부상을 보고하지 않았거나, 연구원이 알아차리지 못했을 가능성을 생각해볼 수 있다. 10년 전과 비교하여 머리부상을 예방하는 방향으로 개정된 야구 룰(예: 홈플레이트 충돌, 빈볼로 인한 퇴장 등)도 영향을 미쳤을 가능성이 있다.

농구

농구는 하체 부상(48건)이 전체부상(70건)의 68.6%를 차지하였는데, 무릎(13건)과 발목(14건)관절의 빈도가 높았다. 그 외 부상으로 발/발가락(7건, 10%), 허리(6건, 8.6%), 아랫하체(6건, 8.6%), 어깨와 팔꿈치(각 4건, 5.7%), 그리고 허벅지(4건, 5.7%) 순이었다. 부상의 형태로 인대염좌(25건, 35.7%)가 가장 높았고, 타박상(16건, 22.9%)이 두 번째로 많았다. 이는 점프와 착지하는 과정에서 상대선수의 발을 밟거나 갑자기 방향을 전환해야 하는 종목의 특성과 수비동작이나 공격 동작에서 상대선수와 접촉하는 과정에서 빈번히 발생하는 것에서 비롯되었음을 추측할 수 있다. 미국대학스포츠협회(1988년부터 2004년까지)의 부상조사분석 결과를 보면, 대학농구 공식경기 중 하체부상이 전체부상의 절반(57.9%)이상을 차지하였고, 그 다음 상체(14.1%), 머리/목(13.9%), 그리고 체간/등/허리(11.4%) 순으로 기록되었다. 하체부상 중 인대염좌가 발목(1103건, 26.2%)과 무릎(312건, 7.4%)에서 많은 분포를 보였다(Dick et al., 2007).

럭비

총 236건의 부상이 발생한 럭비는 발목부상(43건, 18.2%), 무릎(27건, 11.4%), 어깨(24건, 10.2%), 손가락(22건, 9.3%), 아랫하체(21건, 8.9%), 그리고 목(17건, 7.2%) 순으로 기록되었다. 부상 형태로 타박상(111건, 47.0%)이 거의 반을 차지하였고, 인대염좌(78건, 33.1%)가 전체부상 형태의 1/3을 차지하였다. 뇌진탕으로 인한 부상(11건, 4.7%)은 전체 6개 종목의 뇌진탕 부상 14건 중 대부분(78.6%)을 차지하였다. 부상 기전에서 알 수 있듯이, 상대 선수의 접촉으로 인한 부상이 전체 부상 236건 중 181건(76.7%)이며, 움직이는 물체나 고정되어 있는 물체와의 접촉으로 인한 부상을 합산하면 203건(86.0%)이나 집계되었다. 접촉 없이 일어난 부상(19건: 8,1%)과 경기장이나 플레이컨디션으로 일어난 부상(14건, 5.9%) 순 이었다. 2005-06년 한 시즌 동안의 남자 럭비선수 31명의 부상을 조사한 결과, 총 447건의 부상이 머리/얼굴(113건, 25.3%%), 어깨(57건, 12.8%), 무릎(55건, 12.3%), 발목(55건, 11.2%), 팔/팔꿈치(24건, 5.4%), 그리고 손(22건, 4.9%) 순으로 집계되었다. 부상 형태는 인대염좌(102건, 22.8%)가 가장 높았고, 근육/건염좌(71건, 15.9%), 뇌진탕(49건, 11.0%), 골절(36건, 8.1%), 찰과상(35건, 7.8%), 그리고 탈골(18건, 4.0%) 순이었다고 보고하였다(Kerr et al., 2008). 본 연구와 비교해 보면, 발목, 무릎 그리고 어깨의 부상이 순서는 다르지만, 높은 부상 부위로 집계된 점은 비슷한 경향으로 보인다. 본 연구의 머리/얼굴 부상은 24건(10.1%)으로 가장 많은 부상은 아니었던 점이 선행연구와 다른 점이다. 본 연구와 선행연구 모두 뇌진탕은 높은 비율을 차지하고 있어, 즉각적인 대처법과 지침이 있는 미국의 경우와 같이(Harmon et al., 2013; Paddack et al., 2016), 국내에서도 선수들의 뇌진탕 처치 매뉴얼이 필요할 것이다.

태권도

태권도는 가장 긴 시즌(11개월), 가장 많은 선수(51명), 그리고 가장 많은 부상(310건)을 기록한 종목이다. 부상 부위는 허벅지(103건, 33.2%), 무릎(84건, 27.1%), 발/발가락(37건, 11.9%), 그리고 발목(23건, 7.4%) 순으로, 대부분(247건, 79.6%)이 하체부상 이었다. 접촉 종목답게 부상 형태로는 타박상(168건, 54.2%)이 가장 높았고, 인대염좌(32건, 10.1%), 근육통(27건, 8.5%), 근/건 염좌(26건, 8.2%), 그리고 피부조직부상(19건, 6.0%) 순으로 조사되었다. 태권도 종목의 메타분석 연구에서(Lystad et al., 2009), 하체부상(40.2%)과 머리와 목 부상(24.0%) 빈도가 높았고, 부상 형태로는 타박상(36.0%), 인대염좌(12.5%), 찰과상(9.1%), 골절(5.4%) 순으로 기록되어 본 연구와 매우 비슷한 경향을 보였다고 판단된다. 선행연구에서 부상 형태의 한 부분으로 뇌진탕(5.4%)을 보고되었는데, 본 연구에서는 뇌진탕 기록이 없었다. 그 이유로 보통 대학 태권도 겨루기선수들은 훈련 혹은 공식경기에서 머리 공격을 쉽게 허용하지 않으며, 공격을 당해도 정확하게 가격당하는 것은 드물고 대부분 스쳐 점수를 주는 것이 일반적이기 때문으로 추측된다.

배구

배구는 6개 스포츠 종목 중 시즌(5/11-10/20, 163일)이 가장 짧았는데, 시즌이 가장 긴 태권도(2/2-12/14, 316일)보다 153일 적었다. 총 59건의 부상이 발생하였고, 부상 부위에서 손가락 부상(18건, 30.5%)이 가장 높았으며, 발목(10건, 17.0%), 무릎(6건, 10.2%), 어깨(4건, 6.8%), 그리고 얼굴(4건, 6.8%) 순으로 집계되었다. 부상 형태는 인대염좌(32건, 54.2%)가 반 이상을 차지했고, 다음으로 타박상(7건, 11.9%), 근육통(5건, 8.5%), 그리고 만성건염(4건, 6.8%) 순이었다. 종목의 특성 상 스파이크나 블록 시 전해지는 타격이 손가락 부상 빈도를 높였고,점프와 착지가 빈번한 종목의 특성이 무릎과 발목의 부상빈도를 높인 것으로 보인다. 1988년부터 2004 까지)미국대학스포츠협회에서 조사한 연구(Agel et al., 2007)에 따르면, 배구 공식경기 중 하체부상(58.7%)이 가장 많았고, 다음으로 상체부상(21.4%), 체간/등/허리(10.8%), 그리고 머리/목(6.7%) 순으로 나타나 본 연구와 비슷한 결과를 보였다. 본 연구의 주요 부상 부위가 발목, 무릎, 그리고 손가락 이라는 점 또한 선행연구의 부상패턴과 같다.

제한점

조사된 6개 종목은 각각 그 고유의 성격이 있다. 예를 들어, 야구, 농구, 럭비, 그리고 태권도는 선수 간 접촉이 있는 스포츠이고, 배드민턴과 배구는 접촉이 드물거나 거의 없는 스포츠이다. 이들 종목 간에도 접촉의 빈도와 정도는 차이가 있기 때문에 전체로 집계된 부상<Tables 2-5>을 우리나라 대학스포츠의 부상통계라고 보기 힘든 점은 제한점이다. 본 연구에 참여한 종목 개수, 한 종목의 선수숫자, 그리고 수집기간 역시 전체 대학스포츠의 부상통계를 설명하기는 힘들다. 대학스포츠에서 가장 빈번한 부상 형태나 부상 부위를 다른 아마추어 집단(고등학교 혹은 사회인)이나 프로집단과 비교하는 것은 의미가 있다고 보았을 때, 본 파일럿 데이터의 수집/분석/보고 방법을 토대로 향후 더 많은 대학과 종목을 대상으로 장기간(2-3년 이상)의 데이터를 수집해야 할 것이다.

시즌과 비시즌의 정의가 종목마다 달라 그 기간이 다른 점도 제한점이다. 본 연구는 2015년 1월 1일부터 데이터 수집을 시작하였기 때문에 1월 중에 공식경기가 있는 종목들은 그렇지 않은 종목들에 비해 시즌 전 기간이 짧다. 예를 들어, 태권도는 겨울방학 중에 시합이 있었기 때문에 시즌 기간이 316일인 반면, 배구는 5월 중순에 첫 공식경기를 했으므로(메르스 영향) 시즌 기간이 163일 이었다. 미국의 경우 대부분의 종목이 봄이나 가을학기 개강과 맞추어져 있고 비시즌 중 연습을 할 수 있는 시간도 정해져 있어 종목 간 시즌/비시즌의 시간노출(TE)이나 선수노출(AE)이 어느 정도 표준화되어있는데 반해, 우리나라는 종목 간 차이가 커서 이 부분이 전체통계에 영향을 준 것을 감안해야 한다.

결 론

본 연구는 대학 학생운동선수의 부상을 실시간으로 집계하고 분석한 우리나라의 첫 데이터이다. 한 대학 내 6개 종목(배드민턴, 야구, 농구, 럭비, 태권도, 그리고 배구) 167명의 선수를 대상으로 2015년 한 시즌동안 총 961건의 부상이 기록되었다. 부상위험은 시즌 중 공식경기에서 가장 높았으며, 시즌 후 훈련이 가장 낮았다. 부상 부위로는 무릎과 허벅지 부상이 가장 많았으며, 발목, 손가락, 아랫 하체, 발/발가락, 그리고 어깨/쇄골 순으로 빈번하게 부상을 입은 것으로 나타났다. 주요 진단은 타박상이 가장 많았으며, 인대염좌, 피부조직부상, 근/건 염좌, 만성건염, 골절, 뇌진탕, 충돌, 그리고 관절염/활액막염/활액낭염 순으로 나타났다. 본 연구의 수집, 분석, 그리고 보고방법은 향후 국내에서 진행될 부상기록시스템의 빅데이터 구축을 위한 파일럿 데이터로 활용될 것이며, 부상관리/예방 프로그램의 개발에 기초자료를 제공할 것으로 기대한다.
