Abstract

목적

본 연구는 골다공증-근감소성 비만(Osteosarcopenic obesity; OSO) 고령여성의 생리적 특성, 기능체력, 등속성 근기능 특성을 분석하고, OSO집단의 아이리신과 아디포카인, 골대사지표의 관련성을 조사하는 것이다.

방법

본 연구는 60세 이상 고령여성 126명을 대상으로 골다공증, 근감소증, 비만의 기준으로 3가지 표현형을 모두 가진 고령여성 10명(OSO집단: 76.9±5.2 yrs)과 3가지 표현형을 모두 가지지 않는 고령여성 14명(NOSO집단: 72.9±5.6 yrs)으로 구분하여 체격, 신체구성과 골밀도(Hologic, QDR- 4500W, USA), 기능체력(Senior fitness test), 등속성 근력(Cybex 770, USA), 영양소 섭취량(CAN-Pro, version 4.0, Korea), 신체활동량(IPAQ)을 측정·분석하였다. 생화학적 변인으로는 아이리신(Irisin), FNDC5, 렙틴(Leptin), 아디포넥틴(Adiponectin)과 골대사지표(Osteocalcin, CTx, 25(OH)D, PTH)를 비교·분석 하였다. 집단 간 차이를 규명하기 위하여 독립 t-검증을 실시하였으며 OSO집단의 Irisin 수준에 영향을 미치는 생화학적 변인을 분석하기 위하여 stepwise 다중회귀분석을 실시하였다.

결과

신장은 OSO집단이 유의하게 낮게 나타난 반면, 허리둘레, 엉덩이둘레, 허리-엉덩이 비율(WHR), 체질량지수(BMI)는 NOSO집단보다 유의하게 높게 나타났다. 지방조직과 체지방률에서는 OSO집단이 NOSO집단보다 유의하게 높았으며 요추 골무기질량과 골밀도에서는 NOSO집단보다 유의하게 낮았다. 기능체력은 악력과 2-min step test에서 OSO집단이 NOSO집단보다 유의하게 낮았다. 등속성 근력은 60°/sec에서 최대근력은 좌·우측 굴곡근에서 OSO집단이 유의하게 낮았으며, 체중 당 최대근력과 평균파워는 우측 굴곡근, 좌측 신전근, 좌측 굴곡근에서 유의하게 낮게 나타났다. 180°/sec에서 최대근력, 체중 당 최대근력, 총 일량 모두 좌측 굴곡근에서 OSO집단이 유의하게 낮게 나타났다. 영양소 섭취량에서는 단백질섭취량이 OSO집단이 NOSO집단보다 유의하게 낮았으며 신체활동량은 OSO집단에서 낮은 경향을 보였다. 렙틴의 경우 OSO집단이 NOSO집단보다 유의하게 높게 나타났으며 OSO집단의 Irisin 수준에 영향을 미치는 생화학적 변인으로는 Adiponectin, FNDC5, Osteocalcin이 유의하게 영향을 미치는 것으로 나타났다.

결론

결론적으로 OSO집단이 대사증후군, 노화, 골절 등 위험에 더 노출되어 있는 것으로 판단되며, non dominant leg의 대퇴사두근과 대퇴이두근의 불균형으로 인한 낙상위험 증가와 일상생활과 관련된 기능체력, 이동성에 제한이 있는 것으로 사료된다. 향후 OSO의 특징을 더 명확히 규명하기 위한 cohort 연구와 운동프로그램 적용을 통한 종단연구가 진행되어야 할 것으로 판단된다.

Trans Abstract

Purpose

The purpose of this study were to assess physiological and biochemical characteristics in elderly women with osteosarcopenic obesity (OSO), and to analyze relationships among irisin, adipokines and bone metabolism markers.

Methods

126 elderly women were selected and among them 10 women were classified into OSO group (76.9±5.2 yrs) and 14 women were classified as a NOSO group (72.9±5.6 yrs). Physique, body composition and bone mineral density were measured. Senior fitness tests were 30-s chair stand, 30-s arm curl, chair sit-and-reach, back scratch, 8-foot up-and-go, grip strength, and 2-min step test. Isokinetic muscle strength was measured by isokinetic dynamometer (Cybex 770, USA). Nutrition intake and physical activity were administered. Biochemical parameters including irisin, FNDC-5, leptin, adiponectin, CTx, 25(OH)D, osteocalcin, and PTH were measured. All data were analyzed by SAS 9.4. Independent t-test was applied to compare between OSO and NOSO groups. Multiple regression analysis was used. The level of significance was set at .05.

Results

The results of the study showed that there were significantly high for waist circumference, hip circumference, WHR, and BMI in OSO group compared to those of NOSO group. Higher results were also obtained for fat tissue and percent body fat but significantly low for lumbar bone mineral density. OSO group showed significant lower results for grip strength and 2-min step test compared to NOSO group. Peak torque, and relative peak torque at 60° were significantly lower for left and right knee flexion in OSO group. Protein intake was significantly low in OSO group, but no difference was obtained in level of physical activity between two groups. Irisin was significantly related to adiponectin, FNDC-5 and osteocalcin in explaining 35.2%, 81.5% and 92.1% of the variance, respectively.

Conclusions

This study shows that elderly women with OSO have higher results for physique and body composition parameters except body height. However, lower values were obtained for functional fitness, and isokinetic muscle strength. OSO may have more risks for metabolic syndrome, bone fractures, fall, lack of daily physical activity and limit of locomotion due to the imbalance of quadriceps and biceps femoris in non dominant leg. This study suggests that criteria and mechanism of OSO should be clarified by follow-up study.

서론

골다공증-근감소성 비만(osteosarcopenic obesity; OSO)은 비만인 중 낮은 골량 및 골밀도 그리고 낮은 근육량이 동시에 발생하는 질환으로(Ormsbee et al., 2014) 최근, 고령에 따른 비만, 골다공증, 근감소증이 동시에 발현되는 고령 여성이 급격히 증가하여 국가적, 사회적 관심이 급증하고 있다(Ilich et al., 2014; Ormsbee et al., 2014; Kim, 2015). 연령이 증가하면서 신체구성(근육, 지방, 뼈)에 영향을 미치는 다양한 생리학적 변화를 가져온다(Ilich et al., 2014). 근육조직은 30~40세 사이에 최고 수준에 도달한 후 점차 감소하며, 몇몇 개인은 70~80세에 근육량이 약 40%까지 감소한 것으로 보고되었고(Cruz-Jentoft et al., 2010), 이러한 상태를 근감소증이라고 명명하였다(Rosenberg, 2011). 이와 유사하게 뼈 조직은 30세에 최고 수준에 도달한 후 고원상태를 유지하다 점차 연령이 증가함에 따라 감소한다(Matkovic et al., 1994), 특히 여성은 폐경기 이후 골 감소 비율이 급격히 빨라지며(1~2%/년), 골감소증, 골다공증이 남성보다 유병률이 빠르게 나타난다(Kelly et al., 2009). 한편 지방조직은 연령이 증가함에 따라 지방축적이 증가하고 초 고령자의 경우 고원현상을 유지하거나 감소하는 경향을 보였다(Kelly et al., 2009). 특히 고령과 함께 지방은 복부내부(내장지방)로 재분배되고 근육과 뼈에서 지방 침습이 일어난다. 이러한 결과는 근력과 기능체력을 감소시키며, 낙상과 골절, 잠재적인 사망률을 증가시킨다(Cruz-Jentoft et al., 2010). 따라서 고령인구가 증가하고 있는 현시점에서 비만, 골다공증, 근감소증의 증가로 인한 의료비 지출이 증가할 것으로 예상되며 사회적, 정부적 차원에서 대책마련이 시급할 것으로 사료된다.

고령자의 근감소성 비만(sarcopenic obesity; SO)은 세계적으로 약 30% 이상의 높은 유병률을 나타내며 우리나라에서도 1998년 30.2%에서 2009년 37%로 발병률이 증가하고 있다(Loenneke & Pujol, 2011; Cho et al., 2013). 이는 신체활동의 부족, 좌식생활 의 증가, 불균형한 영양소 섭취 등을 포함하여 테스토스테론, 성장호르몬, 염증성 호르몬, 마이오카인 등의 생화학적인 변화가 그 원인이 될 수 있다(Baumgartner, 2000; Weinheimer et al., 2010). 이러한 결과는 근피로, 허약, 생활습관과 관련된 질병을 초래하고(Ambrose et al., 2013), 대사증후군의 위험률을 증가시킨다(Khamseh et al., 2011). 또한 근감소성 골감소증/골다공증(sarco-osteoporosis)은 성호르몬, IGF -1 및 성장호르몬의 변화, 낮은 신체활동량과 단백질 섭취, 비타민 D 결핍이 주요 원인이며, 만성적 염증상태로부터 야기되는 이화작용은 낮은 골밀도와 관련이 있다(Kaji, 2013). 또한 악력은 골다공증과 매우 밀접한 관련이 있고(Sjöblom et al., 2013), 근감소성 골감소증/골다공증은 낙상 및 골절의 위험률을 증가시켜 삶의 질 저하, 질환률과 사망률을 증가시킨다(Binkley & Buehring, 2009). 최근 근육, 뼈, 지방조직 사이에 상호작용이 있으며 골다공증-근감소성 비만은 기능적 장애와 낙상, 골절, 사망률의 위험요인을 증가시키는 것으로 보고되었다(Ilich et al., 2014). 이와 같이 비만, 골다공증, 근감소증은 상호 공통요소가 존재하며 각각의 질환을 분석하는 것도 중요하지만 하나의 질환요소로 결합시켜 살펴보는 것이 낙상 및 골절 등을 더욱더 잘 예측할 수 있으며, 연령의 증가와 함께 나타나는 신체구성 변화에 따른 임상적 결과 역시 잘 반영할 수 있을 것으로 사료된다(Ilich et al., 2014; Ormsbee et al., 2014). 그러나 고령여성의 골다공증-근감소성 비만에 대한 연구는 매우 미흡하며 비 골다공증-근감소성 비만 고령여성(Non-osteo sarcopenic obesity; NOSO) 간 생리적, 기능적 차이를 규명한 연구는 부족한 실정이다.

근육은 근세포에 의해 사이토카인과 다른 펩타이드를 생산하고 분비하는데 이것을 마이오카인(myokine)이라 하며(Pedersen & Febbraio, 2008), 생화학 분야 연구에서는 근세포가 다수의 마이오카인을 분비한다고 보고하고 있다(Bortoluzzi et al., 2006; Henningsen et al., 2010). 그 중 아이리신(irisin)은 운동에 의해 유발되며 피하지방조직에서 지방의 갈색화와 UCP1의 발현을 자극한다. 그 결과 총 에너지 소비량을 증가시키고, 비만과 연관된 인슐린 저항성을 개선시킨다. 따라서 아이리신은 근 활동과 운동으로 인해 정적 자극을 받는 요인이며(Bostrӧm et al., 2012), 향후 에너지 소비에 중요한 역할을 하는 근육과 지방간의 관련성을 규명하는데 중요한 역할을 할 것으로 사료된다(Kim, 2015). 또한 아이리신을 매개로 하여 조골세포 생성의 활성화가 관찰된 바 있어(Colaianni et al., 2013), 근육과 뼈조직에서 발현되는 호르몬 간 상호 관련성이 있을 것으로 예측된다. 지방에서는 렙틴(leptin), 아디포넥틴(adiponectin) 등을 분비하며 비만인에게서 렙틴 수준이 높고 아디포넥틴 수준은 낮다고 보고하였다(Lecke et al., 2011). 또한 골 재형성에 영향을 주며, 렙틴은 교감신경계를 통해 골형성을 억제하는 것으로 알려져 있어(Gimble et al., 1996) 비만은 골감소증/골다공증과 관련이 있다는 것을 설명하고 있다. 이처럼 근육, 뼈, 지방에서 분비되는 호르몬간의 상호관련성이 보고되었지만, 골다공증-근감소성 비만 고령여성을 대상으로 그 관련성을 규명한 연구는 부족한 실정이다.

따라서 본 연구는 골다공증-근감소성 비만(OSO) 고령여성의 생리적, 생화학적 특성을 분석하고, OSO집단의 아이리신과 아디포카인, 골대사지표의 관련성을 조사하여 골다공증-근감소성 비만 고령여성의 특징과 향후 운동처방을 위한 기초자료를 제시하고자 한다.

연구방법

연구대상

본 연구는 G도 Y시에 거주하는 60-85세 고령여성126명을 대상으로 골다공증-근감소성 비만(OSO) 집단과 비 골다공증-근감소성 비만(NOSO) 집단으로 구분하였다. OSO집단의 선정기준은 체지방률이 30%이상 (Okorodudu et al., 2010)이고 요추 혹은 대퇴경부 혹은 대퇴말단 골밀도의 T-score가 <-2.5(Kanis, 1994)이며 근감소증에 해당하는 자로 하였다. 근감소증은 European Working Group on Sarcopenia in older People(EWGSOP)의 알고리즘과 선행연구의 기준치를 이용하여 보행속도 ≤1.26 m/s, SMI(골격근지수) ≤25.6(사지 골격근의 양/체중 X 100), 악력≤20kg(Baumgartner et al., 1998; Cruz-Jentoft et al., 2010; Kim et al., 2012; Ishii et al., 2014)에 해당한 대상자이었다. NOSO집단은 비만, 골다공증, 근감소증 모두 해당하지 않는 자로 하였으며 기능체력 측정이 가능한자로 하였다. 연구대상자의 제외기준은 1) 6개월 이내에 주 2회(최소 1회 20분) 이상 규칙적인 운동 프로그램에 참여한 자, 2) 연구시작 2개월 이내에 경구혈당 강하제, 고혈압 약제 및 콜레스테롤 저하제의 용량에 변화가 있는 자, 3) 1개월 이내 5% 이상의 체중 변화가 있는 자, 4) 의사소통에 문제가 있는 자, 5) 불안정한 협심증(흉통), 통제할 수 없는 고혈압, 제 1형 당뇨병, 심장마비, 심장 수술, 만성신부전증, 간질, 경련, 만성장염 질환 등의 병력과 정신병적 질환으로 인한 심각한 기능 손상이 있는 자로 하였다. 본 연구는 K대학교 생명윤리심의위원회(KHU IRB 2014-G18)에서 승인을 받은 후에 진행하였으며, 모든 대상자는 연구 시작 전 연구내용을 충분한 설명을 듣고 검사 동의서를 받은 후 연구에 참여하였다. 본 연구의 최종 대상자는 OSO 10명, NOSO 14명이었으며 대상자 모집과정은 <Fig. 1>과 같다.

측정항목 및 방법

체격

대상자의 신장과 체중은 신장계(STDK model 1, HD, Japan)와 표준 체중계(Seca Co, USA)를 이용하여 측정하였고, 체질량지수(body mass index; BMI)를 산출하였다. 허리둘레는 12번 늑골과 장골 중간부위를 줄자를 이용하여 측정하였고 0.1cm 단위로 기록하였다. 엉덩이 둘레는 엉덩이 최대 돌출부위를 측정하여 0.1cm 단위로 기록하였다. 지방분포도의 지수인 허리-엉덩이 둘레 비율(waist-hip ratio; WHR)을 산출하였다.

신체구성과 골밀도

신체구성과 골밀도 측정은 Dual X-ray Absorptiometry (Hologic, QDR-4500W, USA)를 이용하여 지방조직(fat tissue; kg), 제지방조직(lean tissue; kg), 체지방률(% body fat; %)을 측정하였다. 골무기질량(bone mineral content; BMC) 및 골밀도(bone mineral density; BMD)는 전신(total), 대퇴(femur), 요추(lumbar), 전완(forearm) 부위를 Scan하여 분석한 후 g/㎠ 단위로 기록하였다.

기능체력

Rikli & Jones(2013)이 고안한 Senior Fitness Test(SFT) 방법을 적용하여 근력, 심폐지구력, 유연성, 민첩성/동적평형성을 측정하였으며, 근력은 하지근력(30초 의자에서 앉았다 일어서기, 회/30초), 상지근력(30초 5lb 아령들기, 회/30초), 악력(kg)을 실시하였다. 심폐지구력은 2분 제자리 걷기(회/2분)를 실시하였으며, 유연성은 상지유연성(등 뒤로 양 손 잡기, cm), 하지유연성(의자에 앉아 윗몸앞으로 굽히기, cm)를 실시하였다. 민첩성/동적평형성은 2.44 m 왕복 걷기(초)를 실시하였다.

등속성 근력

등속성 근력검사는 Isokinetic dynamometer(Cybex 770, USA)를 이용하여 무릎부위의 신근력(extention)과 굴근력(flexion)을 측정하였다. 최대근력을 측정하기 위해 각속도 60°/sec에서 대퇴신근과 대퇴굴근의 근력을 5회 측정하여 그중 가장 높은 Peak torque 값을 기록하였으며, 근지구력 측정은 각속도 180°/sec에서 15회 신전과 굴곡 운동 시 총 일량을 기록하였다.

영양소 섭취량

영양소 섭취량은 24시간 회상법(dietary record method)을 이용하여 일일 에너지 섭취량을 조사하였다. 영양 설문지는 해당 1주일 중 주중 2회, 주말 1회 총 3일치의 섭취한 음식을 연구 대상자가 직접 기록하게 하였고, 각 개인이 작성한 식사일지를 근거로, 한국영양학회에서 제작한 보조영양분석프로그램(Computer Aided Nutritional Analysis Program: CAN-Pro, version 4.0)을 이용하여 총열량섭취량과 3대 영양소 섭취량을 분석하였다.

신체활동 수준

신체활동 수준은 국제 신체활동 설문지(International Physical Activity Questionnaire; IPAQ)를 이용하여 대상자의 신체활동량을 측정하였다. 자가보고형 설문지를 이용하여 조사한 신체활동량은 Craig et al.(2003)의 연구에 제시된 MET(Metabolism)로 환산한 후 분석하였으며 환산한 공식은 다음과 같다.

  • ∙걷기 MET-minutes/week = 3.3 x 걷기 시간 x 걷기 횟수

  • ∙중강도 MET-minutes/week = 4.0 x 중강도 신체활동 시간 x 중강도 신체활동 횟수

  • ∙고강도 MET-minutes/week = 8.0 x 고강도 신체활동 시간 x 고강도 신체활동 횟수

총 신체활동량은 걷기, 중강도, 고강도의 신체활동량을 합산하여 ‘MET-min/week’로 산출하였다(IPAQ, 2004).

생화학적 변인

혈액채취는 대상자가 12시간 공복상태에서 측정 당일 오전 8시 검사 장소에 도착하여 30분간 안정을 취하도록 한 후, 대상자의 상완 주정맥(antecubital vein)에서 1회용 주사기를 이용하여 20 ml의 정맥혈을 채취하였다. 채취된 혈액은 각 분석항목에 따라 항응고 처리 및 처리되지 않은 튜브에 넣어 3,000 rpm에서 10분간 원심분리한 후, 세포성분(cellular elements)을 제외한 혈장(plasma)과 혈청(serum)을 뽑아 –80 ℃로 냉동 보관하였다. 아이리신은 Assay kit(Phoenix Pharmaceuticals, #EK-067-16, Burlingame, CA), FNDC5는 Assay kit(Phoenix Pharmaceuticals, #EK-067-19, Burlingame, CA)를 이용하여 효소면역분석법(enzyme immunoassay; EIA)으로 분석하였고, 렙틴은 Leptin kit(USA), 아디포넥틴은 Adiponectin kit(USA)를 이용하여 전기화학발광면역측정법(electro chemiluminescence immunoassay; ECLIA)으로 분석하였다. 오스테오칼신은 immulite 2000 Osteocalcin(USA), 25(OH)D는 Liaison 25- (OH) vitamin D Total Kit(Diasorin, USA)를 이용하여 화학발광면역측정법(chemiluminescent immunoassay; CLIA)으로 분석하였다. CTx는 Serum Coss Laps ELISA kit(Immunodiagnostic systems Ltd., AZ)를 이용하여 효소결합면역흡착분석법(enzyme- linked immunosorbent assay; ELISA)으로 분석하였으며, PTH는 Elecsys PTH Kit(Roche, Germany)를 이용하여 전기화학발광면역측정법(electro chemiluminescence immunoassay; ECLIA)으로 분석하였다. 모든 생화학적 변인은 서울시 S 임상검사센터에 의뢰하여 분석하였다.

자료처리 방법

자료처리는 SAS software version 9.3(SAS Institute, Cary, NC)을 이용하여 분석하였다. 모든 자료는 평균(mean)과 표준편차(standard deviation ; SD)로 제시하였다. 집단 간 차이를 검증하기 위하여 독립 t-검정(Independent t-test)를 실시하였으며 아이리신 수준에 영향을 미치는 생화학적 변인을 분석하기 위하여 stepwise 다중회귀분석을 실시하였다. 회귀모형의 설명력은 수정된 R2 값을 제시하였으며, 모든 통계처리에 대한 유의수준(α)은 .05로 하였다.

연구결과

신체적 특성

OSO집단과 NOSO집단의 신체적 특성을 비교 분석한 결과는 <Table 1>에 제시된 바와 같다. 신장의 경우 OSO집단이 NOSO집단보다 유의하게 낮게 나타났다(p<.05). 한편, 허리둘레, 엉덩이 둘레, WHR에서 OSO집단이 NOSO집단보다 유의하게 높게 나타났으며(p<.05), 체질량지수(Body Mass Index; BMI)도 유의하게 높게 나타났다(p<.01).

신체구성 및 골무기질량, 골밀도

OSO집단과 NOSO집단의 신체구성과 골무기질량, 골밀도를 비교 분석한 결과는 <Table 2>에 제시된 바와 같다. 지방조직과 체지방률의 경우 OSO집단이 NOSO집단보다 유의하게 높게(p<.001) 나타난 반면 제지방조직에서는 두 집단 간 유의한 차이가 없었다. 요추 골무기질량과 요추 골밀도의 경우 OSO집단이 NOSO집단보다 유의하게 낮게 나타났다(p<.001). 한편, 전신 무기질량 및 골밀도, 대퇴 무기질량 및 골밀도, 전완 무기질량 및 골밀도에서는 집단 간 유의한 차이가 나타나지 않았다.

기능체력

OSO집단과 NOSO집단의 기능체력을 비교 분석한 결과는 <Table 3>에 제시된 바와 같다. 악력(Grip strength)(p<.001)과 심폐지구력(2min-step test)의 경우 OSO집단이 NOSO집단보다 유의하게 낮게 나타났다(p<.01). 한편 하지근력(30-s Chair stand test), 상지근력(30-s Arm curl), 하지유연성(Chair sit-and-reach), 상지유연성(Back scratch), 민첩성/동적평형성(8-foot up-and-go)에서는 집단 간 유의한 차이가 나타나지 않았다.

등속성 근력

OSO집단과 NOSO집단의 등속성 근력(60°/sec)을 비교 분석한 결과는 <Table 4>에 제시된 바와 같다. 최대근력(Peak torque)의 경우 우측 굴곡근(p<.01)과 좌측 굴곡근(p<.001)에서 OSO집단이 NOSO집단보다 유의하게 낮게 나타났다. 또한, 체중 당 최대근력(Peak torque %body weight)에서는 우측 굴곡근(p<.01), 좌측 신전근(p<.05), 좌측 굴곡근(p<.001)에서 OSO집단이 NOSO집단보다 유의하게 낮게 나타났으며, 평균파워(Average power)에서도 우측 굴곡근(p<.05), 좌측 신전근(p<.05), 좌측 굴곡근(p<.01)이 유의하게 낮게 나타났다. OSO집단과 NOSO집단의 등속성 근력(180°/sec)을 비교 분석한 결과는 <Table 5>에 제시된 바와 같다. 최대근력, 체중 당 최대근력, 총 일량(Total work done) 모두 좌측 굴곡근(p<.05)에서 OSO집단이 NOSO집단보다 유의하게 낮게 나타났다.

영양소 섭취량 및 신체활동량

OSO집단과 NOSO집단의 영양소 섭취량과 신체활동량을 비교 분석한 결과는 <Table 6>에 제시된 바와 같다. 단백질(Protein)의 경우 OSO집단이 NOSO집단보다 유의하게 낮게 나타났으며(p<.05), 총 에너지 섭취량(Total energy intake), 탄수화물(Carbohydrate), 지질(Lipid)에서는 집단 간 유의한 차이가 나타나지 않았다. 또한 신체활동량(Physical activity)에서도 두 집단 간 유의한 차이가 나타나지 않았다.

생화학적 변인

OSO집단과 NOSO집단의 생화학적 변인을 비교 분석한 결과는 <Table 7>에 제시된 바와 같다. 렙틴(Leptin)의 경우 OSO집단이 NOSO집단보다 유의하게 높게 나타났으며(p<.01), 아이리신(Irisin), FNDC-5, CTx, 아디포넥틴(Adiponectin), 비타민 D(25(OH)D), 오스테오칼신(Osteocalcin), 부갑상선 호르몬(PTH-intect)에서는 두 집단 간 유의한 차이가 나타나지 않았다.

아이리신과 생화학적 변인의 단계별 다중회귀분석

OSO집단의 아이리신 수준과 생화학적 변인간의 관련성을 분석하기 위하여 다중회귀분석을 실시한 결과는 <Table 8>에 제시된 바와 같다. 아이리신 수준은 1단계 아디포넥틴(β= .651, p<.05), 2단계 아디포넥틴(β= .694, p<.01), FNDC-5(β= -.658, p<.01), 3단계에서는 아디포넥틴(β= .604, p<.001), FNDC-5(β= .635, p<.001), 오스테오칼신(β= .316, p<.05)이 유의하게 영향을 미치는 것으로 나타났으며, 전체 설명력은 92.1%(F= 36.128)로 나타났다.

논의

본 연구는 OSO와 NOSO의 신체구성, 기능체력, 영양섭취량과 신체활동량, 아이리신 및 아디포카인과 골대사지표의 차이를 분석하고 아이리신 수준에 영향을 미치는 생화학적 변인을 분석하고자 실행하였다. 본 연구에서는 126명의 고령여성 중 OSO의 발생률은 약 12%(15명)로 나타났으며, Ilich et al.(2015)의 연구에서 보고한 OSO의 발생률 12.4%와 유사한 것으로 나타났다. 그러나 Kim et al.(2016)에서는 25% 발생율을 보고하여 본 연구보다 매우 상회하였고 이러한 차이는 연구자간 각각 다른 근감소증의 진단기준에 의한 것으로 사료된다. 즉 선행연구에서는 근육량을 이용하여 근감소증을 진단한 반면 본 연구에서는 EWGSOP 알고리즘에 따라 근육량과 기능체력(악력, 걷기속도) 평가를 이용했기 때문에 발생한 것으로, 아직까지 근감소증 진단기준과 관련된 연구가 초기단계에 있어 추후 연구가 실행되어야 할 것으로 사료된다.

OSO집단과 NOSO집단의 체격을 비교한 결과 신장, 허리둘레, 엉덩이둘레, WHR, BMI에서 유의한 차이가 나타났으며 신체구성에서는 체지방량, 체지방률에서 유의한 차이가 나타났다. 이러한 차이는 예견된 결과로서 본 연구에서 OSO집단과 NOSO집단을 골다공증, 근감소증, 비만의 기준을 이용하여 대상자를 분류하였기 때문인 것으로 사료된다. Ilich et al.(2015)의 연구에서 OSO집단, 근감소성 비만(SO)집단, 골감소성 비만(osteopenic obesity; OO)집단, 비만(obese only; OB)집단을 비교한 결과 OSO집단과 SO집단이 가장 높은 체지방률을 보고하여 본 연구결과와 일치하고 있다. 또한 근육량에서는 OO집단과 OB집단에 비해 사지의 근육량이 유의하게 낮게 나타났다. 그러나 본 연구에서 두 그룹간 근육량은 유의한 차이가 나타나지 않았으나 OSO집단이 낮은 경향을 보였다. 그 이유는 NOSO집단의 근육량이 근감소증 기준에 근소한 차이로 도달하지 못하였으며 OSO특성상 대상자 수가 많지 않기 때문에 나타난 결과로 사료된다. 또한 본 연구에서 요추의 골무기질량과 골밀도에서 OSO집단이 유의하게 낮게 나타났다. 40세 이상 여성을 대상으로 2년간 추적 조사한 결과 연령의 증가, 폐경, 높은 체지방률이 요추 골밀도 손실의 위험요인이라 보고하여(Kim et al., 2015) 본 연구 결과를 뒷받침한다. 선행연구에서 지방과 근육은 신체질량을 구성하는 요인으로 체중이 많을수록 골무기질량에 미치는 물리적인 부하가 커지기 때문에 비만-골다공증, 그리고 비만-근감소증은 각각 상호 배타적인 관계라고 인식하였다(Rosen & Bouxsein, 2006; Ilich-Ernst et al., 2002). 하지만 최근 선행연구에서 과체중/비만, 특히 체지방은 뼈에 긍정적이지 않다고 보고하여(Ilich et al., 2014), 골감소증/골다공증이 포함된 신체구성 표현형에 대한 추후 연구가 이루어져야 할 것으로 사료된다.

기능체력에서는 OSO집단이 NOSO집단에 비해 낮은 경향을 보였으며 특히 악력(근력)과 2분 제자리걷기(심폐지구력)에서 유의한 차이를 보였다. 기능체력은 대사증후군 지표와 관련이 있으며(Vieira et al., 2013; Mileski et al., 2015), 대사적 이상 징후를 예측하기 위한 유용한 검사이다(Aparicio et al., 2014). 악력은 고령자의 정적근력을 평가하는 예측인자이며 악력의 감소는 낙상 위험률 증가와 깊은 관련이 있음을 보고하였다(Savino et al., 2013; Cruz-Jentoft et al., 2014). 또한 높은 심폐체력은 비만과 관련된 사망 위험율의 증가를 감소시키는 반면에 임상적 관점에서 심폐체력의 중요성이 간과되고 있다(Lee et al., 2010). 따라서 본 연구에서 나타난 OSO집단의 낮은 체력수준은 추후 삶의 질과 고령자의 독립성을 더욱 약화시켜 대사증후군 위험이 증가할 것으로 사료되며 기능체력 향상을 위한 트레이닝이 필요하다.

등속성 근력은 60°/sec의 모든 변인에서 좌·우측 굴근력의 유의한 차이가 나타났으며, 체중 당 최대근력과 평균파워에서는 좌측 신근력에서 유의한 차이가 나타났다. 한편 180°/sec의 모든 변인에서는 좌측 굴근력에서 유의한 차이가 나타났다. Marcell et al.(2014)의 연구에서는 48세~64세 여성의 굴근력이 4.7년 후 유의하게 감소하였으며 1년마다 3~4%의 근력 손실이 일어났다고 보고하였다. 또한 대퇴이두근과 대퇴사두근의 비율에 의해 하지의 잠재적 상해율을 증가시키며(Hewett et al., 2008), 근력의 불균형은 낙상의 강한 예측인자라고 보고하였다(Skelton et al., 2002). 특히 대퇴이두근은 하지의 한쪽 근력이 감소했을 때 계단오르기나 걷기에 중요한 역할을 한다(Valtonen et al., 2015). 본 연구에서 주목할 점은 OSO집단이 NOSO집단에 비해 주로 사용하지 않는 다리의 굴근력이 유의하게 낮아 대퇴이두근과 대퇴사두근의 뷸균형이 심각한 것으로 해석할 수 있다. 이는 보행, 계단 오르기와 같은 일상생활을 위한 이동성에 제한을 주어 낙상이나 골절과 같은 상해의 위험성이 OSO집단에서 더 크게 나타난 것으로 사료된다.

영양소 섭취량과 신체활동량을 비교분석한 결과 단백질에서 OSO집단이 유의하게 낮게 나타났으며 총 에너지 섭취량과 신체활동량은 유의한 차이는 없으나 낮은 경향을 보였다. Kim et al.(2016)의 연구에서는 OSO집단의 총 에너지 섭취량과 단백질 섭취량이 정상집단과 다른 표현형을 가진 집단보다 낮게 나타나 본 연구 결과와 일치하고 있다. 그러나 Ilich et al.(2015)의 연구에서는 OSO, OO, OB 집단보다 SO집단에서 체중 당 단백질 섭취량이 가장 낮게 나타나 본 연구와 상이한 결과를 보고하였으나 근감소증을 가진 집단에서 단백질 섭취가 부족하다는 것이 공통된 결과이다. Layman et al.(2003)은 단백질 섭취는 근육량의 손실을 예방하고 근력 및 근지구력을 개선시키기 때문에 매우 중요하다고 보고하였다. 신체활동량 부족은 지방세포의 증가를 가져와 염증상태를 유발하여 골격근의 기능과 그 크기에 악영향을 미친다(Meyer et al., 2011; Issa & Griffin, 2012). 또한 골격근의 양과 기능은 좌식생활과 깊은 관련이 있으며 좌식생활은 근감소증 비만을 유발한다(Li & Heber, 2012; Malafarina et al., 2012). 따라서 고령여성에게 단백질 섭취 및 신체활동량의 증가는 중요한 것으로 판단되며 개인적, 사회적 관심이 필요할 것으로 사료된다.

본 연구에서 생화학적 변인을 분석한 결과 렙틴에서만 유의한 차이가 나타났다. 젊은 여성의 아디포넥틴 수준은 높고 렙틴 수준이 낮게 나타나지만 연령이 증가할수록 지방축적에 따라 상반된 현상을 보인다(Lecke et al., 2011). 근육량의 증가는 렙틴의 증가를 가져오기 때문에 낮은 렙틴 수준은 근감소증의 지표가 될 수 있다고 제안되었으나(Hubbard et al., 2008) 근감소증 비만, 골다공증-근감소성 비만에게 과도한 지방세포로부터 분비된 렙틴은 예외일 수 있다(Ilich et al., 2014). 본 연구에서 OSO집단의 렙틴은 유의하게 높았으나 아디포넥틴은 낮은 경향을 보여 선행연구와 일치하고 있다. 따라서 이러한 결과는 OSO집단이 NOSO집단보다 체지방률과 체지방량에서 유의하게 높았기 때문에 나타난 결과로 사료된다. 아이리신과 FNDC5의 경우 유의한 차이는 나타나지 않았으나 OSO집단이 낮은 경향을 보였다. Hou et al.(2015)의 연구에 따르면 건강한 성인과 비만 성인의 아이리신 수준을 비교한 결과 비만 성인이 유의하게 더 낮았다고 보고하였으며, 아이리신 수준은 BMI, WHR, 지방량과 부적관계가 있다고 보고하였다(Moreno-Navarrete et al.,2013). 그러나 Huh et al.(2012)는 근육량이 아이리신 수준의 예측인자라고 보고하였다. 따라서 본 연구에서 아이리신 수준의 차이가 없었던 것은 제지방량의 차이가 나타나지 않은 결과로 사료된다. 골대사지표인 오스테오칼신, CTx, 25(OH)D, PTH를 분석한 결과 집단 간 차이가 나타나지 않았다. Snijder et al.(2005)에 따르면 높은 BMI와 허리둘레는 낮은 25(OH)D와 유의하게 관련이 있으며, 체지방률은 25(OH)D와 부적관계, PTH와 정적관계가 있다고 보고하였다. 또한 낮은 25(OH)D와 높은 PTH는 노쇠와 관련이 있다고 보고하였다(Tajar et al., 2013). 본 연구에서 OSO집단과 NOSO집단의 25(OH)D와 PTH에서 유의한 차이는 없지만 선행연구와 유사한 경향을 보였다. 따라서 OSO집단의 이러한 경향은 추후 노쇠로 이어질 수 있으며 그 대책을 마련해야 할 것으로 사료된다.

OSO집단의 아이리신 수준에 영향을 미치는 생화학적 변인을 분석한 결과 아디포넥틴, FNDC5, 오스테오칼신이 아이리신 수준에 영향을 미치는 것으로 나타났다. 아디포넥틴은 조골세포와 근세포에서 수용체가 발현되며 지방세포에서 분비한다(Lecke et al., 2011). 근육에서 아디포넥틴은 지방산화, 미트콘드리아 수를 증가 시키며(Lee & Shao, 2012), 조골세포 생산을 촉진한다(Oshima et al., 2005). 또한, 오스테오칼신은 인슐린 민감도와 분비 조절, 지방 산화뿐만 아니라 아디포넥틴의 합성을 자극한다고 보고하였다(Lee & Karsenty, 2008). 아이리신 수준은 골다공증성 골절과 관련이 있으며(Palermo et al., 2015; Anastasilakis et al., 2014), 아이리신이 조골세포의 증식과 분화를 촉진한다고 보고하였고(Qiao et al., 2016). 근육 내에서 흰색지방세포의 열 발생 과정을 활성화한다(Mahajan & Patra, 2013). 따라서 근육, 뼈, 지방에서 분비되는 호르몬간의 관련성이 있으며, 추후 연구에서는 분자수준의 연구를 통한 기전연구가 이루어져야 할 것으로 사료된다.

결론 및 제언

본 연구의 목적은 OSO와 NOSO집단의 체격, 신체구성, 기능체력, 영양소 섭취량과 신체활동량과 생화학적 변인을 비교분석하고, OSO집단의 아이리신과 아디포카인, 골대사지표와 관련성을 규명하는 것이다. 연구결과를 요약하면 OSO집단이 허리둘레, 엉덩이둘레, WHR, BMI, 체지방 및 체지방률이 더 높으며 요추골밀도와 골무기질량이 유의하게 낮게 나타났다. 기능체력은 대부분 낮은 경향을 보였으며 특히 근력과 심폐지구력에서 유의하게 낮게 나타났다. 등속성 근기능에서는 60°/sec에서 최대 근력은 좌·우측 굴곡근, 체중 당 최대근력과 평균파워에서는 우측 굴곡근, 좌측 신전근, 굴곡근에서 OSO집단이 유의하게 낮게 나타났다. 180°/sec에서는 모든 항목에서 좌측 굴곡근이 유의하게 낮게 나타났다. 영양소섭취량은 단백질에서 OSO집단이 유의하게 낮게 나타났으며, 신체활동량은 낮은 경향을 보였다. 생화학적 변인의 경우 렙틴에서 집단 간 유의한 차이가 나타났으며 OSO집단의 아이리신 수준은 아디포넥틴, FNDC5, 오스테오칼신이 영향을 미치는 것으로 나타났다. 본 연구결과를 종합하여 볼 때 OSO집단이 비만, 근감소증, 골다공증과 관련된 대사증후군, 노화, 골절 등의 위험에 노출되어 있으며, 특히 non dominant leg의 대퇴이두근과 대퇴사두근의 불균형을 보여 낙상위험증가와 일상생활을 위한 기능체력, 이동성에 제한을 줄 것으로 사료된다. 또한 신체활동량과 식이섭취를 개선시키고 낙상위험 및 상해예방을 위한 운동프로그램 개발이 필요하다. 추후 연구에서는 OSO집단의 특징을 명확히 규명하기 위한 cohort 연구와 운동처치 및 식이섭취를 통한 효과를 규명하기 위해 종단연구가 필요할 것으로 사료된다.
